手法复位石膏托外固定治疗桡骨远端骨折 83 例疗效观察

目的探讨观察手法复位石膏托外固定治疗桡骨远端骨折的临床疗效。方法对2010年1月-2012年6月收治的83例桡骨远端骨折应用手法复位石膏托外固定治疗的患者进行临床观察，采用Dienst腕关节评分进行功能评价。结果全部83例患者随访11～40月，平均17．8个月。术后腕关节功能优良率：Fernandez分型Ⅰ型骨折95．5％，Ⅱ型骨折50％，Ⅲ型骨折18．2％，Ⅳ型和Ⅴ型骨折0．0％。结论手法复位石膏托外固定治疗Fernandez分型Ⅰ型桡骨远端骨折效果满意，而对于非Ⅰ型骨折疗效不理想，临床治疗中应当慎重选择。

Clinical Therapeutic Effects of Manual Reduction under Traction and Plastic Splint Fixation in the Treatment of 83 Cases with Distal Radius Fractures

Objective To observe the clinical therapeutic effects of manual reduction under traction and plastic splint fixation in the treatment of 83 cases with distal radius fractures. Methods From January, 2010 to June, 2012, 83 patients with distal radius fractures treated with manual reduction under traction and plastic splint fixation were observed. The wrist joint function was evalu- ated according to the Dienst scoring system. Results The 83 cases had complete follow-up records and a mean follow-up of 17.8 months （range, 11 to 40 months）. The excellent and good rate of wrist joint function after the operation by dienst standard was as follows： Fracture classification by Fernandez I is 95.5%, Fernandez II is 50%, Fernandez III is 18.2%, Fernandez IV and V is 0%. Conclusion Manual reduction and plastic splint fixation for treatment with distal radius fractures of Fernandez I have significant efficacy, but can not get the satisfying results for the rest types. We should choose the appropriate therapy according to different cases in the clinical work.
