电刺激督脉穴位对脑梗死后肢体痉挛、运动功能的影响

摘    要:目的:观察电刺激督脉穴位对脑梗死患者肢体痉挛和运动功能的影响。方法将80例脑梗死患者随机分为电刺激治疗组(40例)和普通治疗组(40例),两组患者均进行常规的药物治疗和基本的康复训练,电刺激治疗组加用督脉穴位(大椎、筋缩、腰阳关和命门)调制中频电刺激,普通治疗组不加任何电刺激,两组均治疗1个月,疗程结束后比较两组临床总有效率及采用Fugl-Meyer运动功能评分(FMA)、改良Ashworth评级及改良Barthel指数(MBI)进行运动功能、痉挛程度和生活能力的评定。结果:电刺激治疗组和普通治疗组的总有效率分别为95.0%和75.0%,两组总有效率比较差异均有显著性(P<0.05);电刺激治疗组和普通治疗组治疗后改良Ashworth评级、Fugl-Meyer运动功能评分及改良Barthel指数评分均较治疗前显著改善,治疗前后比较有差异(P<0.05);电刺激治疗组上述各指标改善幅度比较显著,与普通治疗组比较,组间差异具有统计学意义(P<0.05)。结论:督脉穴位电刺激可降低肢体的肢体肌张力,改善痉挛状态,提高肢体的运动功能及改善日常生活能力。

Abstract:Objective:To observe the electrical stimulation of acupuncture point in Du meridian on limb spasticity in patients with cerebral infarction and motor function.Methods:80 patients with cerebral infarction were randomly divided into the electrical stimulation treatment group(40 cases)and conventional therapy group(40 cases),patients in both groups were given routine drug treatment and rehabilitation of basic training,electrical stimulation treatment group was stimulated points in Du meridian(Dazhui,Jinsuo,Yaoyangguan and Mingmen)with modulation frequency electrical stimulation,conventional therapy group was without any stimulation,patients were treated for 1 month.After treatment clinical total effective rate and the Fugl-Meyer motor function score(FMA),modified Ashworth rating and improvement of Barthel Index(MBI)for motor function,spasticity,and the assessment of life skills were compared.Results:The total effective rate of electrical stimulation group and the conventional therapy group were 95.0% and 75.0% respectively,the total effective difference between the two groups was significant(P<0.05);Ashworth rating,Fugl-Meyer motor function score and the modified Barthel Index score of electrical stimulation treatment group and the treatment group were improved than those before treatment and were significantly improved after treatment,the comparison was different(P<0.05);these indicators of electrical stimulation treatment group improved significantly,and compared with conventional therapy group,there was statistically significant differences between groups(P<0.05).Conclusion:The effect of electroacupuncture stimulation can reduce the body’s physical muscle tension,improve spasticity and improve motor function and improve the physical activities of daily living.
