子宫肌瘤妇女微创剔除效果研究

目的 探讨子宫肌瘤妇女微创剔除的临床治疗效果.方法 收集2015年6月至2016年6月份来十堰市太和医院救治的子宫肌瘤患者148例为研究对象.将所选的研究患者分为两组,即腹腔镜组与开腹组.腹腔镜组患者下给予腹腔镜肌瘤剔除术,开腹组患者给予开腹肌瘤剔除术,比较两组患者的临床资料及术后相关指标.结果 腹腔镜肌瘤剔除术患者的手术时间、出血量、排气时间、开始下床活动时间均明显低于开腹肌瘤剔除术患者(t值分别为2.451、2.631、2.042、2.185,均P＜0.05).腹腔镜肌瘤剔除术患者术后总并发症的发生率明显低于开腹肌瘤剔除术患者(x2=5.613,P＜0.05).腹腔镜肌瘤剔除术患者的住院时间明显短于开腹肌瘤剔除术的患者(t=1.993,P＜0.05);而术后6个月后患者的肌瘤残留率及复发率均略高于开腹组患者,但无统计学的差异(x2值分别为1.422、2.021,均P＞0.05).结论 微创肌瘤剔除术具有手术和住院时间短、术中出血量少、开始下床和术后排气时间早及术后并发症少等优点,值得在临床工作中大力推广应用.

Effect of minimal invasive myomectomy

Objective To explore the clinical therapeutic effect of minimal invasive myomectomy.Methods Altogether 148 patients with uterine fibroid hospitalized in Taihe Hospital of Shiyan City from June 2015 to June 2016 were selected as research objects and divided into two groups,laparoscopy group and laparotomy group.Patients in laparoscopy group were treated with laparoscopic myomectomy,while patients in laparotomy group received abdominal myomectomy.Clinical data and postoperative indexes of patients in two groups were compared.Results Operation time,bleeding amount,exhaust time and time to get out of bed in laparoscopy group were significantly lower than those in laparotomy group (t value was 2.451,2.631,2.042 and 2.185,respectively,all P ＜ 0.05).Incidence of postoperative complications in laparoscopy group was significantly lower than that in laparotomy group (x2 =5.613,P ＜ 0.05).Length of hospital stay of patients with laparoscopic myomectomy was significantly shorter than that of those with abdominal myomectomy (t =1.993,P ＜ 0.05).Residual and recurrence rates in six months after operation in laparoscopy group were slightly higher than those in laparotomy group,but there was no statistically significant difference (x2 value was 1.422 and 2.021,respectively,both P ＞ 0.05).Conclusion Minimal invasive myomectomy has advantages of short operation time,short hospital stay,less intraoperative blood loss,shorter exhaust time,earlier to get out of bed and less complications,which is worth to be promoted and applied in clinical practice.
