香砂六君子汤联合西药治疗HP阳性脾胃虚弱型消化性溃疡临床研究

目的:运用中药香砂六君子汤联合西药治疗幽门螺杆菌(HP)阳性脾胃虚弱型消化性溃疡的效果。方法:前瞻性选取84例HP阳性脾胃虚弱型消化性溃疡患者,西医组给予常规四联疗法治疗,联合组运用四联疗法联合中药香砂六君子汤治疗,观察两组HP清除率、临床疗效、中医症状积分和血清一氧化氮(NO)、白介素-6(IL-6)水平变化。结果:两组治疗后上腹痛、胃胀、反酸、烧心4项主要症状积分均明显下降(P<0.05),联合组治疗后上述中医症状积分显著低于西医组(P<0.05)。联合组HP根除率90.48%、总显效率85.71%均高于西医组73.81%、66.67%(P<0.05)。两组治疗后血清NO、IL-6水平均明显降低,联合组治疗后血清NO、IL-6水平显著低于西医组(P<0.05)。结论:四联疗法联合香砂六君子汤能提高HP阳性脾胃虚弱型消化性溃疡疗效和HP根除率,显著降低血清NO、IL-6水平,体现出中西医结合疗法的临床优势。

Clinical study of Xiangsha Liujunzi decoction combined with western medicine in the treatment of HP positive peptic ulcer

Objective:To explore the effect of the treatment of peptic ulcer with HP positive spleen stomach weakness of Xiangsha Liujunzi decoction combined with western medicine.Methods:84 patients with HP positive peptic ulcer of spleen stomach weakness type were selected prospectively.Western medicine group was treated with conventional quadruple therapy.The combination group was treated with quadruple therapy and Xiangsha Liujunzi decoction.The HP clearance rate,clinical effect,symptom score and blood were observed.The levels of nitric oxide(no)and interleukin-6(IL-6)in serum were changed.Results:After treatment,the scores of the four main symptoms in the two groups decreased significantly(P<0.05),and the scores of the above symptoms in the combined group were significantly lower than those in the western group(P<0.05).The eradication rate of HP in the combined group was 90.48%,and the total effective rate was 85.71%,which was higher than that in the western medicine group(73.81%,66.67%).The difference was statistically significant(P<0.05).Serum NO and IL-6 water were decreased significantly after treatment.The levels of serum NO and IL-6 in the combined group were significantly lower than those in the western medicine group(P<0.05).Conclusion:Silian therapy combined with Xiangsha Liujunzi decoction can improve the curative effect and HP eradication rate of HP positive peptic ulcer with weakness of spleen and stomach,significantly reduce the level of serum NO and IL-6,reflecting the clinical advantages of the combination of traditional Chinese and Western medicine.
