中枢神经系统肠源性囊肿的MR诊断与鉴别诊断

目的探讨中枢神经系统肠源性囊肿的MRI表现及其鉴别诊断。方法回顾性分析7例经手术和病理证实的肠源性囊肿的临床和MRI资料。结果 7例肠源性囊肿,桥前池1例,椎管内髓外硬膜下6例,3例病变位于颈段,2例位于上段胸段,1例位于腰段。6例椎管内肠源性囊肿,5例囊肿位于脊髓腹侧,均可见"脊髓嵌入征"。多数囊肿T1Wl呈等或稍高于脑脊液信号,T2WI等或稍低于脑脊液信号。6例行MR增强扫描,5例囊壁及囊液均未见强化,1例囊壁轻度强化。结论 MRI对肠源性囊肿的诊断及鉴别诊断有重要价值。

Diagnosis and differential diagnosis of enterogenous cyst of central nervous system by MR

Objective To study the MRI manifestations and differential diagnosis of enterogenous cyst of central nervous system.Methods The MRI appearance and clinical features of 7 cases with enterogenous cysts proved by surgery and pathology were collected and analyzed.Results Among 7 cases of enterogenous cysts,1 intracranial case located in anterior pontine cistern,and 6 intraspinal cases located in the subdural extramedullary space,among which,3 lesions occurred in cervical segments,2 lesions in the upper thoracic segments,1 lesions in lumbar segments,6 cysts in the spinal enterogenous,5 lesions localized at anterior to the spinal cord,which all showed sign of spinal cord incuncation.Most of cysts were isointensity or slightly hyperintensity compared to cerebrospinal fluid on T1Wl,Isointensity or slightly hypointensity to cerebrospinal fluid on T1WI.6 lesions were performed with enhanced MR scan,5 of them had enhancement in bursa wall and bursa liquid,1 slightly enhancement in bursa wall.Conclusion MRI is helpful in diagnosis and differential diagnosis of the enterogenous cyst.
