公立医疗机构的产业内集聚研究

目的研究我国医院体制改革与医疗资源及服务利用产业内集聚的关系。方法回顾医院体制改革的相关文件,利用历年财务年报对医疗机构的自主化经营和医疗服务的市场化进行实证分析;度量医疗资源和服务利用产业内集聚的情况和历年来的变化趋势,进而考察这种集聚现象与医疗机构民营化之间的关系。结果医疗资源和服务逐步聚集到了大型医疗机构,无论是从政策路径还是从实证分析上看,我国的公立医疗机构都走上了事实上民营化的道路,这与医疗资源和服务的高位流向密切相关。结论医疗机构事实上的民营化是医疗资源及服务产业内集聚的原因,解决医疗资源产业内聚集的问题需要政府予以基层卫生服务更大的投入。

Research on the inner-industrial agglomeration of public hospitals

Objective To explore the relationship between hospital system reform and the inner-industrial agglomeration of health resources and services utilization. Methods To review the policy documents of hospital reform,do empirical study on hospital autonomous management and marketization of health services,and go further to analyze the relationship between hospital privatization and its inner-industrial agglomeration. Results Public hospitals are under privatization,medical resources and services are concentrating in large hospitals,and this two issues are highly correlated. Conclusions The actual privatization of public hospitals is one reason of the inner-industrial agglomeration of health resources and services utilization.To solve the inverse triangle health resource allocation problem,more resources are needed from the government to support the development of community health services.
