外周置入中心静脉导管置管长度测量方法的可行性

摘    要:目的:探讨外周置入中心静脉导管置管长度测量方法的可行性。方法:选择本院肿瘤科行PICC置管患者108例,进行置管长度测量方法和导管X线定位的探讨。结果:导管管端至右胸锁关节下缘的长度(c值)为(47.90±10.96)mm,第6胸椎右侧上缘至右胸锁关节下缘的长度(D值)为(48.06±11.14)mm,差异无统计学意义(t=0.362,P=0.718),本方法可行。结论:放置PICC管其末端均位于第6胸椎上缘周围,以右胸锁关节下缘至右第3胸肋关节下缘的长度的1/2加上穿刺点至右胸锁关节下缘的长度可作为外周置入中心静脉导管的置管体外测量长度。

Abstract:Objective:To investigate the feasibility of a novel method that measures the inserted length of peripherally inserted central catheter ( PICC ). Method : 108 patients who received placement of PICC in our Department of Oncology were enrolled for measurement of inserted length by using the tested method and X-ray evaluating methods of measuring length of the catheter and catheter X-ray localization. Results: The distance between the tip of catheter and the inferior margin of right sternoclavicular joint ( C value) was (47.90 ± 10.96) mm vs (48.06 ± 11.14)mm for the distance between the right upper margin of sixth thoracic vertebra and the inferior margin of right sternoclavicular joint ( D value), with no significant difference between both (t = 0. 362, P =0. 718). Accordingly we considered it feasible to use the tested method for measuring the insert length of PICC. Conclusion:The tip of inserted PICC is consistently located in the adjacency of upper margin of the 6th thoracic vertebra. Therefore it is feasible to determine the insert length of PICC by adding half of distance between the inferior margin of right sternoclavicular joint and inferior margin of the 3rd sternocostal joints to the length between puncture point and the inferior margin of right sternoclavicular joint.
