基于医院信息系统数据的预算趋势分析及评估研究

目的:基于医院信息系统(HIS)数据,研究趋势分析及评估模式在医院预算管理中的应用价值。方法:选取2015-2018年医院设备购置预算、设备维修保养预算、信息化建设预算和临床科研预算4个归口的预算管理数据,将2015-2016两个年度的预算管理数据纳入对照组,采用传统的预算审批和考核模式管理,将2017-2018两个年度的预算管理数据纳入观察组,采用预算趋势分析及评估模式管理。预算执行数据通过HIS进行采集,临床满意度采用自制满意度测评问卷调查临床医务人员对预算项目临床应用的满意度,对比分析两组预算执行率、临床效益指数和临床满意度的差异性。结果:观察组和对照组的预算执行率分别为93.95%(2048/2180)和92.51%(1889/2042),观察组预算执行率高于对照组,其差异有统计学意义(x^2=6.227;P<0.05);观察组和对照组的临床效益指数分别为(120.1±6.18)%和(113.9±11.39)%,临床医务人员对预算项目的满意度分别为(92.1±3.80)%和(88.2±3.94)%;观察组的临床效益指数和临床满意度明显高于对照组,其差异有统计学意义(t=-2.484,t=-3.230;P<0.05)。结论:基于HIS数据建立的趋势分析及评估模式,可降低医院运行成本,优化临床资源配置,提高医院预算管理水平。

Research on trend analysis and assessment about the budget on basis of the data of clinical information platform

Objective:To explore the application value of trend analysis and assessment model in budget management of hospital based on the data of hospital information platform(HIS).Methods:The budget management data of four centralized project budgets of hospital,namely equipment purchase,equipment maintenance,informatization construction and clinical research,from January 2015 to December 2018 were selected as the research objects.And the budget management data of 2015 year and 2016 year were included in control group,and the traditional management model of budget approval and appraisal was implemented on them.The budget management data of 2017 year and 2018 year were included in observation group,and the management mode with trend analysis and assessment of budget was implemented on them.The budget enforcement data were collected by HIS,and the self-made satisfaction questionnaire was used to investigate the clinical satisfaction of clinically medical staffs about the clinical application of budget item.The differences of budget enforcement rate,clinical benefit index and the degree of clinical satisfaction between the two groups were compared and analyzed.Results:The budget enforcement rates of observation group and control group were 93.95%(2048/2180)and 92.51%(1889/2042),respectively.And budget enforcement rate of observation group was significantly higher than that of control group(x^2=6.227,P<0.05).In observation group and control group,the clinical benefit indexes were(120.1±6.18)%and(113.9±11.39)%,and the degrees of clinical satisfaction were(92.1±3.80)%and(88.2±3.94)%,respectively.The clinical benefit index and the degree of clinical satisfaction of observation group were significantly higher than those of control group(t=-2.484,t=-3.230,P<0.05),respectively.Conclusions:The trend analysis and assessment modelbased on the data of HIS can reduce the operating cost of hospital,and optimize the allocation of clinical resources,and effectively improve the budget management level of hospital.
