BRAFT1799A基因突变甲状腺乳头状癌超声特征

摘    要:目的研究甲状腺乳头状癌(PTC)组织中BRAFT1799A突变与结节超声特征之间的关系。方法选择行甲状腺切除的PTC病人160例,应用聚合酶链反应及DNA直接测序法检测新鲜组织标本中BRAFT1799A基因突变情况,结合甲状腺超声检查结果分析BRAFT1799A基因突变与结节边界、钙化、血流、淋巴结转移及结节大小等超声特征的关系。结果160例PTC病人中,BRAFT1799A突变阳性者46例,突变率为28.75%。BRAFT1799A突变阳性病人血流丰富的比例大于阴性者(x2=4.88,P〈0.05);Logistic回归分析显示,血流丰富与BRAFT1799A突变呈正相关(0R=3.368,P〈0.01);BRAFT1799A突变阳性组与阴性组其他超声特征比较差异无显著性(P〉0.05)。结论PTC组织中BRAFT1799A基因突变与结节血流关系密切,而与其他超声特征无明显相关性。

Abstract:Objective To investigate the correlation between BRAF T1799A gene mutation in tissue of papillary thyroid carcinoma (PTC) and the ultrasound features of the thyroid. Methods One hundred and sixty patients with PTC underwent thyroidectomy were enrolled in this study. Using polymerase chain reaction (PCR) and direct DNA sequencing, the BRAF T1799A gene mutation in fresh tissue samples was detected, which was analyzed by comparing with ultrasonographic results in terms of the boundary of nodules, calcification, blood supply, lymph node metastases and tumor size. Results Of the 160 patients,46 were BRAF T1799A positive, mutation rate being 28.75M. Those of positive BRAF T1799A showed a higher ratio of rich blood supply of their tumor samples than the negative ones (X2 = 4.88, P %0.05). Logistic regression showed a positive correlation between BRAF T1799A mutation and rich blood flow (OR =3.368,P%0.01). The chi-square test showed no significant differences between BRAF T1799A mutation positive and negative groups in other sonographic features (P~0.05). Conclusion BRAF T1799A mu- tation in PTC is closely associated with the blood flow around the nodules but not with other sonographic features.
