婴幼儿体外循环前后ICAM-1的变化及意义

摘    要:目的:研究婴幼儿心脏手术体外循环(CPB)前后心肌细胞间黏附分子-1(ICAM-1)的变化及其意义.方法:30例室间隔缺损(VSD)患儿,按有无肺动脉高压(PH)分为无PH组(A组)15例,PH组(B组)15例.于CPB前和CPB结束即刻分别取心肌检测ICAM-1含量.结果:两组患儿心肌ICAM-1的含量均于CPB后明显升高(P<0.01).B组心肌ICAM-1的含量高于A组(P<0.01).结论:先心伴PH患儿CPB后心肌ICAM-1的含量明显高于先心不伴PH的患儿.ICAM-1在CPB后心肌缺血再灌注损伤中起重要作用.

Abstract:Objective:To investigate change and significance of intercellular adhesion mol-ecule-1(ICAM-1) on myocardium before and after cardiopulmonary bypass(CPB) in infants cardiac surgery. Methods: Thirty children with ventricular septal defect(VSD) were enrolled in this study.They were devided into two groups: no-PH group (group A) and PH group(group B). The expression of ICAM-1 on myocardium was assayed before and after CPB. Results: The expression level of ICAM-1 on myocardium increased significantly after CPB in two groups(P<0.01). The expression level of ICAM-1 on myocardium in group B was significantly higher than that of group A after CPB(P<0.01). Conclusion:The expression level of ICAM-1 on myocardium in congenital heart disease kids with PH is significantly higher than that in congenital heart disease kids without PH. ICAM-1 may play important roles in myocardial ischemia-reperfusion injury after CPB.
