经颅多普勒超声评价椎动脉切迹频谱与其病变部位的关系

摘    要:目的探讨经颅多普勒超声(TCD)所提示的椎动脉颅内段频谱呈切迹改变者与其病变部位的相关性。方法采用TCD筛选出椎动脉颅内段频谱呈切迹改变者24例,并经数字减影血管造影(DSA)或CT血管成像(CTA)、颈部血管彩色多普勒血流成像(CDFI)检查进一步明确其病变部位。结果椎动脉多普勒频谱呈舒张期末.收缩期前宽切迹组(9例):DSA或CTA示患侧椎动脉起始段重度狭窄5例、闭塞2例、先天发育纤细2例;其中同侧锁骨下动脉正常8例,合并同侧锁骨下动脉中度狭窄1例,合并对侧锁骨下动脉闭塞1例;血流动力学试验阴性。椎动脉多普勒频谱呈收缩期切迹组(12例):根据血流动力学试验结果分为阳性组6例,DSA示椎动脉起始段正常,锁骨下动脉均有不同程度的病变;阴性组6例,DSA示椎动脉起始段闭塞4例、先天发育纤细2例;其中合并同侧锁骨下动脉狭窄2例,同侧锁骨下动脉正常4例。椎动脉多普勒频谱振荡小尖波组(3例):DSA示椎动脉起始段闭塞3例,其中1例合并同侧锁骨下动脉闭塞,经颈部肌支盗血,而无椎动脉-椎动脉盗血,同侧锁骨下动脉正常2例;血流动力学试验阴性。结论椎动脉血流频谱切迹不仅见于锁骨下动脉盗血综合征早期,还可以出现在锁骨下动脉病变但盗血途径不完善及椎动脉起始段自身病变的患者中;血流动力学试验可助于鉴别。TCD可用于锁骨下动脉及椎动脉自起始段至颅内段病变的初步诊断,但有一定的局限性,需结合DSA、颈部CDFI分析。
