高龄贲门胃底癌经腹手术治疗的评价(附84例)

目的：探讨高龄患者胃底贲门癌的外科治疗，总结高龄贲门胃底癌患者手术治疗的经验。方法：回顾性分析高龄贲门胃底癌84例手术治疗情况，平均年龄69．5（65～84）岁。插管全麻下，经腹行贲门癌根治性切除术；围手术期予营养支持、呼吸道管理。结果：术后发生并发症14例（16．7％），其中呼吸道并发症6例（42．9％）；死亡0例。随访58例，平均随访3．5（1～7）年，3年生存率为70．7％（41／58），5年生存率为53．4％（31／58）。结论：贲门胃底癌经腹手术可提同高龄患者的生存率和生活质量，取得满意疗效。

Abdoimal surgical treatment for fundus venticuli or cardiac cancer of 84 advanced aged patients

Objective:To study the surgical therapy for the aged patients suffered from fundus venticuli or cardiac cancer,and explore the surgical approach.Methods:Total of 84 aged cases of fundus venticuli or cardiac cancer underwent radical surgery were retrospectively reviewed.Agverage age was 69.5.Under general anesthesia with bilateral tracheal intubation,patients were treated through abdominal approach.Postoperative treatment included nutritional supplement and respiratory management.Results:Complications developed in 14 cases(16.7%),including respiratory complications in 6 cases(42.9%) no deaths occurred.In 1 to 7 years follow-up,the 3,5 year survival rates were 70.7% and 53.4%.Conclusion:The operation is an ideal way to treat the aged patients with fundus venticuli or cardiac cancer and it can improve the patients' quality of life and prolong survival time.
