非体外循环冠状动脉旁路移植术的临床应用

目的总结非体外循环冠状动脉旁路移植术的治疗经验。方法整理并分析2003年5月~2006年2月148例非体外循环冠状动脉旁路移植术的临床资料。其中陈旧性心肌梗塞71例,三支病变110例,左主干病变58例,急诊及亚急诊手术18例。结果全组手术死亡4例。平均移植桥血管3.12±0.66支,采用左乳内动脉147例,选用桡动脉为桥血管48例。术后心绞痛均消失,心脏功能改善。结论非体外循环冠状动脉旁路移植术安全、有效;取得满意手术效果的关键是把握手术适应证、术中配合、手术技巧和围手术期处理。

Clinical application of off-pump coronary bypass grafting

Objective To summarize the experience of the off-pump coronary bypass grafting(OPCAB).Methods One hundred and forty-eight clinical data of patients received OPCAB were sorted and analyzed.71 cases were old myocardial infarction,58 patients were left main lesion and 110 cases were triple vessel disease.Eighteen patients were performed with emergency and urgent operations.Results Four patients died after the operation.The mean number of blood vessel graft was 3.12±0.66.Left internal mammary artery was used for 147 cases and radial artery for 48 cases.The sangina pectoris disappeared after the operation and the cardiac function was improved in all cases.Conclusion OPCAB is a safe and effective measure for patients with coronary artery disease.The key to satisfactory surgical results are including grasping optimal indication,operative cooperation,surgical skills and correct perioperative management.
