Study on the localization of the origin of ventricular tachycardia with surface electrocardiogram

This study evaluated the ability of catheter endocardial pacing at or near the site of origin of spontaneous ventricular tachycardia(VT) to mimic the QRS configuration of the spontaneous VT.Surface electrocardiographic QRS configuration produced by catheter endocardial pacing was compared with the electrocardiographic QRS configuration of spontaneous VT to identify the origin of VT in 8 patients.The success of electric ablation of VT was taken as golden standard.The results showed that when the QRS configuration produced by endocardial pacing was in accordance with that of spontaneous VT in more than 9 electroeardiographic leads.the pacing point was lying at the site in close proximity to the site of originof VT,and the origin of VT could be successfully ablated by direct current but not by radiofrequency current.The results also showed that when the QRS configuration produced by endocardial pacing was in accordance with that of spontaneous VT in all 12 electrocardiographic leads,the pacing point could coincide with the site of origin of VT,and the origin of VT could be successfully ablated by radiofrequency current.The method of localizing the origin of VT we used could be cosidered as accurate and feasible.
