肝切除术联合术中胆道镜治疗肝内胆管结石疗效分析

目的探讨分析肝切除术联合术中胆道镜治疗肝内胆管结石的临床效果。方法对我院采用肝切除术联合术中胆道镜治疗肝内胆管结石86例的疗效进行分析。结果肝切除术后有残余结石27例,结石残余率为31.40%(27/86);术中胆道镜残余结石总取净率为81.48%(22/27)。86例患者胆道镜取石后3例出现发热,无其他并发症。本组中80例术后获得随访,随访时间6～72个月。其中优56例(70.00%),良23例(28.75%),差1例(1.25%),优良率达98.75%。结论肝切除术治疗肝内胆管结石疗效确切,术中行胆道镜检查可以发现术中遗留的肝内胆管结石,联合取石可有效降低外科手术结石残余率。

Therapeutic effects analysis of liver resection combined with intraoperative choledochoscope in treating intrahepatic bile duct stones

Objective To investigate the therapeutic effects of liver resection combined with intraoperative choledochoscope in treating intrahepatic bile duct stones.Methods Therapeutic effects analysis was conducted among 86 patients with intrahepatic bile duct stones who received liver resection combined with intraoperative choledochoscope.Results After treatment,27 patients had retained calculus(the retained calculus rate was 31.40%),in which the total removing rate of the retained calculus by choledochoscope was 81.48%.Three patients suffered from fever,but had no other complications.Postoperative follow-up was performed among 80 patients for 6 to 72 months,among which 56 patients got excellent recovery(70.00%),23 patients got fine recovery(28.75%),1 patient got poor recovery(1.25%).The excellent and fine rates were 98.75%.Conclusions Treating intrahepatic bile duct stones with liver resection has the exact therapeutic effects.The intraoperative retained intrahepatic bile duct stones can be found by choledochoscope.Liver resection combined with intraoperative choledochoscope can reduce the retained stone rate.
