经尿道双极等离子电切术治疗良性前列腺增生症

目的 探讨经尿道前列腺等离子双极电切术治疗良性前列腺增生的临床疗效.方法 选取2014年5月至2015年5月收入本科室治疗的良性前列腺增生症(BPH)患者44例,随机分为观察组和对照组,每组22例.观察组采用经尿道前列腺等离子双极电切术(TUPKP)治疗,对照组采用经尿道前列腺电切术(TURP)治疗.对比两组手术时间、术中出血量、术后膀胱冲洗时间、留置导管时间、住院时间,并发症发生率、IPSS评分、QOL评分、Qmax和RVU等指标.结果 观察组术中出血量、术后膀胱冲洗时间、留置导管时间低于对照组,差异具有统计学意义(P＜0.05);两组手术时间、住院时间差异无统计学意义(P＞0.05).两组患者治疗后症状均明显改善,IPSS评分、QOL评分、残余尿RVU较治疗前明显下降(P＜0.05);最大尿流率Qmax较治疗前明显升高(P＜0.05);术前和术后3个月IPSS评分、QOL评分、Qmax、RVU比较,组间差异无统计学意义(P＞0.05).观察组患者并发症发生率(18.18％)明显低于对照组(59.09％),差异具有统计学意义(P＜0.05).结论 TUPKP治疗BPH具有更好的安全性,值得临床推广应用.

Bipolar transurethral plasmakinetic prostatetomy in the treatment of benign protastic hyperplasia

Objectives To evaluate the clinical efficacy of bipolar transurethral plasmakinetic prostatetomy(TUPKP) in the treatment of benign protastic hyperplasia(BPH).Methods Forty-four patients with BPH in our hospital from May 2014 to May 2015 were randomly divided into observation group and control group,each with 22 cases.The patients in observation group were treated by TUP-KP,and those in control group were treated by transurethral resection of the prostate(TURP).The data including operative time,blood loss,continuous bladder irrigation time,ductal indwelling time,hospitalization time,complications rate,IPSS score,QOL score,maximum flow-rate(Qmax) and residual urine volume(RVU) were compared between the two groups.Results The blood loss,continuous bladder irrigation time and ductal indwelling time in observation group were significantly lower than that in control group(P ＜0.05).The operative time and hospitalization time had no significant difference between the two groups (P ＞ 0.05).After treatment,the IPSS score,QOL score and RVU of two groups reduced variously than before and the Qmax after treatment was significantly higher than that before(P ＜0.05).There were no significant differences in IPSS score,QOL score,Qmax and RVU between the two groups (P ＞0.05).The complications rate in observation group(18.18％)was significantly lower than that in control group (59.09 ％,P ＜ 0.05).Conclusions TUPKP is an effective and safe method for patients with BPH and worthy of clinical application.
