Survivin基因在喉癌组织及声带息肉组织中的表达差异及临床意义

摘    要:目的 探讨Survivin基因在喉癌组织及声带息肉组织中的表达及其与喉癌患者临床病理特征、Caspase-3、p53的关系。方法 收集2017年6月—2019年3月于我院行手术切除的135例原发性喉癌患者癌组织石蜡标本作为研究对象,另选择我院同期收治的100例声带息肉患者的息肉组织标本作为对照组。采用免疫组织化学染色法检测喉癌组织及声带息肉组织中Survivin、Caspase-3、p53的表达情况,PCR检测Survivin、Caspase-3及p53的表达水平。比较Survivin、Caspase-3、p53在喉癌组织及声带息肉组织中的表达及Survivin基因与喉癌患者临床病理特征、Caspase-3、p53的关系。结果 Survivin、p53在喉癌组织中的阳性表达率分别为68.15%、65.93%,均显著高于其在声带息肉组织中的阳性表达率(34.00%、38.00%)(P<0.001)。Caspase-3在喉癌组织中的阳性表达率(35.56%)显著低于其在声带息肉组织中的阳性表达率(77.00%)(P<0.001)。喉癌组织中Survivin、p53的表达水平显著高于声带息肉组织,Caspase-3表达水平显著低于声带息肉组织(P<0.001)。Survivin基因在中低分化、TNM分期Ⅲ-Ⅳ期、无淋巴结转移的喉癌患者中呈高表达(P<0.05)。Spearman相关性分析结果显示,Survivin基因与Caspase-3的表达呈负相关(r=-0.334, P<0.001),与p53的表达呈正相关(r=0.537, P<0.001)。结论 Survivin基因在喉癌组织中高表达,与喉癌患者临床病理特征及Caspase-3、p53等细胞因子关系密切。临床上对Survivin基因进行检测,可为喉癌的诊断、治疗及预后提供一定的指导依据。

Abstract:Objective To investigate the expression of Survivin gene in laryngeal cancer and vocal cord polyp and its relationship with clinicopathological features, Caspase-3 and p53.Methods The paraffin specimens of 135 cases of primary laryngeal cancer patients who had resection surgery in our hospital between June 2017 and March 2019 were collected as the research objects. Another 100 cases of vocal cord polyps during the same period were selected as control study. The expressions of Survivin, Caspase-3 and p53 were detected by immunohistochemical stain and PCR methods. Compare the expressions of survivin, Caspase-3 and p53 in laryngeal cancer and vocal cord polyp, and analyze the relationship of Survivin with the clinicopathological features and expressions of Caspase-3 and p53 in laryngeal cancer.Results The positive expression rate of Survivin was 68.15% in laryngeal cancer tissues , and was 34.00% in vocal cord polyp tissues, while that of p53 was 65.93% in laryngeal cancer tissues and 38.00% in vocal cord polyp tissues. Both were significantly higher in laryngeal cancer tissues than in vocal cord polyp tissues (P<0.001). The positive expression rate of Caspase-3 was significantly lower in laryngeal cancer tissues (35.56%) than in vocal cord polyp tissues (77.00%) (P<0.001). The expression levels of Survivin and p53 in laryngeal cancer tissues were higher than in vocal cord polyps tissues (P<0.001), while the expression level of Caspase-3 in aryngeal cancer tissues was lower than in vocal cord polyp tissues (P<0.001). Survivin gene was highly expressed in patients with clinicopathological characteristics of low differentiation, TNM stage III-IV and no lymph node metastasis (P<0.05). Spearman correlation analysis showed that survivin was negatively correlated with Caspase-3 expression (r=-0.334, P<0.001), and positively correlated with p53 expression (r=0.537, P<0.001).Conclusion Survivin gene is highly expressed in laryngeal cancer, and closely related to the pathological characteristics of laryngeal cancer and the cytokines such as Caspase-3 and p53. The detection of Survivin gene in clinic can provide some guidance for the diagnosis, treatment and prognosis of laryngeal cancer.
