基于真实世界分析速效救心丸治疗冠心病心绞痛的临床用药特征

目的基于真实世界分析速效救心丸治疗冠心病心绞痛的病症特点及临床用药特征。方法从医院信息系统(HIS)中选择全国33家三级甲等医院使用速效救心丸治疗的冠心病心绞痛住院病人信息,采用频数分析法对数据库人群年龄、性别等一般资料进行描述分析;采用Apriori算法建立模型,使用Clementine12.0关联分析病人合并用药情况。结果统计人群中,详细记录年龄的病人共10008例,≥65岁的病人8043例,占80.37%;病人多合并高血压、脑梗死、呼吸系统感染等疾病;速效救心丸主要经口服途径给药,单次剂量≤15粒的病人共9688例,占96.58%;速效救心丸常联合使用的西医药物主要包括硝酸异山梨酯、阿司匹林肠溶片、肝素、美托洛尔、抗生素、胰岛素等。结论心绞痛病人主要集中于老年人,速效救心丸的临床使用基本符合说明书推荐;速效救心丸主要与扩张血管、降血压、抗生素类、胰岛素等药物联合使用,与冠心病病理机制及合并的多种疾病之间存在相关性,提示临床疾病之间互相影响,故选择治疗方案应兼顾多种疾病,以达到最佳疗效。

The Clinical Characteristics of Suxiao Jiuxin Pills in the Treatment of Coronary Heart Disease and Angina Pectoris Based on the Real World Analysis

Objective Based on the real world analysis of the symptoms and clinical characteristics of Suxiao Jiuxin Pills in the treatment of coronary heart disease and angina pectoris.Methods The informations of patients with coronary heart disease and angina pectoris receiving Suxiao Jiuxin Pills in 33 Grade A hospitals in China were selected from the Hospital Information System(HIS),and the general data such as age and gender were described and analyzed by frequency analysis method.The Apriori algorithm was used to establish the model,and Clementine12.0 correlation was used to analyze combined drug use.Results In the statistical population,there were 10008 patients with detailed recorded age and 8043 patients≥65 years old accounted for 80.37%;and most patients with hypertension,cerebral infarction,respiratory system infection and other diseases.Suxiao Jiuxin Pills was mainly administered by oral administration,and 9688 patients with a single dose of less than 15 Pills,accounting for 96.58%.Western medicine used in combination with Suxiao Jiuxin Pills mainly included isosorbide nitrate,aspirin enteric-coated tablets,heparin,metoprolol,antibiotics,and insulin.Conclusion Most patients with angina pectoris were elderly,and they took Suxiao Jiuxin Pills according instructions recommended.Suxiao Jiuxin Pills were mainly used in combination with vasodilators,blood pressure lowering,antibiotics,insulin and other drugs,some of that were correlated with the pathological mechanism of coronary heart disease and a variety of combined diseases,suggesting that some diseases were effected with each other,so the clinical selection of treatment should take into account a variety of diseases in order to achieve the best efficacy.
