摘    要:目的 通过不同透析方式对晚期糖基化终末产物(AGEs)清除作用进行比较,探讨能更好清除中分子毒素的透析方式,指导临床治疗.方法 20例尿毒症患者,接受血液透析滤过(HDF)联合高通量透析(HFHD)10例,接受常规血液透析(CHD)治疗10例,用荧光分光光度计法对透析患者血清AGE8水平进行测定(治疗6个月前后以及单次HDF、HFHD、CHD治疗前后),并对其进行比较分析.结果 单次HDF、HFHD治疗血清AGEs水平均明显下降(P<0.05),但2组间差异无统计学意叉(P>0.05),单次CHD治疗血清AGEs水平差异无统计学意义(P>0.05);治疗后6个月透析前AGEs水平各组差异均无统计学意义(P>0.05).但HDF HFHD组HDF低于CHD组(P<0.05).结论 HDF、HFHD治疗是清除尿毒症患者体内AGEs的一条重要途径;长期联合HFHD对AGEs清除作用明显优于CHD治疗.

Abstract:Objective To analyze the effects of different purification modalities in eliminating advanced glycation end-products(AGEs) in patients with uremia,and to evaluate their related clinical value.Methods 20 uremia patients,who accepted various blood purification modalities were divided into 2 groups as HDF HFHD group(10 cases) and CHD group(10 cases).The AGEs was measured by immunofluorescence spectrophotometry before treatment and 6 months after blood purification,and the related factors were analysed.Results The serum AGEs levels were significantly different between pre-dialysis and post-dialysis in patients treated with HDF and HFHD,while there was no significant difference with CHD treatment(P>0.05).Before and after six months treatment,the levels of AGEs of two groups were no significant difference(P>0.05),while the levels of AGEs in HDF HFHD group were significantly lower than those of CHD group(P<0.05).Conclusion The alteration of serum AGEs are associated with various blood purification methods.The AGEs level is lower in patients treated with long-term HDF HFHD than those treated with CHD,suggesting that HDF and HFHD may be an important dialysis modality in cleaning AGEs in patients with uremia.
