自体骨髓干细胞移植对促进骨折早愈的临床研究

摘    要:目的:研讨自体骨髓干细胞移植治疗四肢长骨骨折的临床疗效。方法:对80例行切开复位内固定术的四肢长骨骨折患者,随机分成干细胞移植治疗组和常规治疗组,进行治疗随访,系统回顾,评价对比疗效。结果:80例患者均获随访12~28个月,平均16个月,干细胞移植组较常规组平均骨痂出现时间提前2.1周,骨折线消失平均时间提早2.7个月,肢体日常功能恢复平均时间提前2个月;常规组出现1例骨折不愈合,2例骨折延迟愈合,干细胞移植组无骨折不愈合和延迟愈合发生;两个治疗组手术中出血量无明显差异,两组均无术后感染病例;干细胞移植组平均手术时间较常规组多12 min。结论:治疗骨折中运用自体骨髓干细胞移植,有较强的成骨作用,能显著提早骨折愈合,取材容易,操作简单,治疗安全,无毒性,规避骨折不愈合发生,提高患者生活质量。

Autogenous bone marrow-derived stem cells transplantation in fractures treatment

Abstract:Objective To investigate the effect of autogenous bone marrow-derived stem cells( BMSCs)transplant for treating frac-ture. Method 80 patients with long bone fractures were divided into 2 groups at random,group of open reduction and internal fixation ( ORIF)and group of ORIF combined with BMSCs transplant,40 patients in each group,fracture healing and functional recovery were eval-uated by X-ray film and follow-up observation. Results All patients were followed up for 12-28 months,average 16 months. Bone callus formation time,bone union time and functional recovery time in BMSCs transplant group were significantly shorter than ORIF group which were 2. 1 weeks,2. 7 months and 2 months respectively. 1 case of nonunion and 2 cases of delayed union of fracture were found in ORIF group,and no such case was found in BMSCs transplant group. No significant difference was found about intraoperative blood loss between the groups,and no infection was found in two groups. The average operation time in transplant group was 12 minutes more than ORIF group. Conclusion The autogenous bone BMSCs transplant shows a good effect for fracture union,which can promote bone repair and regen-eration and has obvious advantages over traditional treatment.
