实时动态胰岛素泵治疗初发2型糖尿病的疗效观察

目的：观察实时动态胰岛素泵（SAP）与单纯胰岛素泵持续皮下输注胰岛素（CSII）治疗初发2型糖尿病的疗效及安全性。方法随机将40例初发2型糖尿病患者分为两组，每组20例。S A P组用722型胰岛素泵持续皮下输注超短效胰岛素及餐前追加大剂量调整血糖，应用实时动态血糖监测系统（RT -CGMS）监测血糖；单纯CSII组用712型胰岛素泵持续皮下输注超短效胰岛素及餐前追加大剂量调整血糖，应用血糖仪监测指尖血糖。结果两组治疗后空腹血糖（FPG ）、餐后2 h血糖（2hPG）均有显著下降，但两组间差异无统计学意义（P＞0．05）；SAP组与单纯CSII组比较，血糖达标时间（3．7±0．4）VS（4．4±0．5），差异有统计学意义（P＜0．001），低血糖发生频次（0．25±0．44） VS ．（0．60±0．50），差异有统计学意义（P＜0．05）。结论 SAP治疗初发2型糖尿病较CSII更快、更安全，在显著降低血糖的同时并不增加低血糖风险。

Observation of effectiveness of sensor-augmented insulin-pump therapy in newly diagnosed type 2 di-abetes

Objective To observe the efficacy and safety of sensor-augmented insulin-pump （SAP） therapy and continuous subcutaneous insulin infusion （CSII） therapy .Methods Forty newly diagnosed type 2 diabetes were randomly divided into two groups and n=20 in each group .The SAP group were infused continuously with short-acting insulin with 722 insulin-pump ,and given additional high-dose before meal ,then the blood glucose was measured by real time continuous glucose monitoring system （RT-CGMS） .CSII group were infused continuously with short-acting insulin with 712 insulin pump , and given additional high-dose before meal ,then the fingertip glucose was measured by blood glucose meter .Results After treatment ,the fasting blood glucose （FPG ） and 2-hour postprandial blood glu-cose （2hPG） decreased significantly ,but there were no significant differences between the two groups ;the therapeutic time was shorter in group SAP than in CSII （P〈0 .001）;the hypoglycemia incidence was lesser in group SAP than in CSII （P〈0 .05） .Conclusion Compared with CSII therapy ,the SAP therapy can control hyperglycemia more quickly and safely ,and didn＇t increase the risk of hypoglyce-mia at the same time .
