不同麻醉方式对老年食管癌患者术后肺部感染的影响

目的:探讨不同麻醉方式对老年食管癌患者术后肺部感染的影响.方法:收集老年食管癌患者80例,根据麻醉方式不同,分为单腔气管内插管全身麻醉组(对照组)和单腔气管内插管全身麻醉复合硬膜外麻醉组(观察组),各40例.观察并记录2组患者的术中出血量、手术时间、术后2d疼痛视觉模拟评分、住院时间、苏醒时间、拔管时间及术后肺部感染情况.结果:与对照组比较,观察组患者术后苏醒时间及拔管时间均较对照组明显缩短(P<0.01),且观察组术后肺部感染发生率低于对照组(P<0.05).结论:全身麻醉复合硬膜外麻醉应用于老年食管癌患者,麻醉效果理想,且较气管插管全麻术后肺部感染发生率更低,值得临床进一步推广应用.

Effect of different anesthesia methods on the postoperative pulmonary infection in elderly patients with esophageal carcinoma

Objective:To explore the effects of different anesthesia methods on the postoperative pulmonary infection in elderly patients with esophageal carcinoma. Methods:Eighty patients with esophageal cancer were divided into the control group and observation group according to the anesthesia method(40 cases each group). The control group and observation group were anesthetized using single lumen endotracheal intubation and single lumen endotracheal intubation combined with epidural,respectively. The amount of bleeding,operation time,postoperative 2 d visual analogue score,hospitalization time,recovery time,extubation time and postoperative pulmonary infection in two groups were observed. Results:Compared with the control group,the recovery time and extubation time in the observation group significantly shortened(P<0. 01),and the incidence of pulmonary infection in observation group was significantly less than that in control group(P <0. 05). Conclusions:General anesthesia combined with epidural anesthesia is suit for elderly patients with esophageal cancer,the anesthesia effect of which is ideal. The incidence of pulmonary infection after general anesthesia with endotracheal intubation is lower,which is worthy of clinical application.
