闭合性十二指肠损伤诊治分析

目的：总结闭合性十二指损伤的诊治经验，以提高诊治水平。方法：回顾分析闭合性十二指肠损伤15例的临床资料。结果：15例中14例合并其他脏器损伤，术前明确诊断1例，术中漏诊3例，术后十二指肠漏3例，胰漏1例，治愈11例，死亡4例。结论：闭合性十二指肠损伤术前确诊困难，术中易漏诊；早期明确诊断，术中仔细探查，选择合适的手术方法，是治疗成功的关键。

An Analysis of the Diagnosis and Treatment of Closed Duodenal Injury

Objestive:To investigate the diagnostic methods and treatment principles of the closed duodenal injure.Method:The diagnosis and treatment of 15 patients with the closed duodeual injury during past 10 years were analyzed retrospectively.Result:14 patients were multiple injury,1 case was diagnosed properly before operation,3 cases were misdiagnosed intraoperatively;There were 3 cases with the duodenal fistula and 1 case with pancreatic fistula after operation;11 cases were cured successfully,4 patients died from the complication.Conclusion:It is very difficult to diagnose the closed duodenal injury preoperatively.The early diagnosis,careful intraoperative examination and proper operative method for the closed duodenal injury are very important for the Successful treatment.
