特发性肺含铁血黄素沉着症1例误诊分析

目的：探讨特发性肺含铁血黄素沉着症患儿的临床诊断、误诊分析及治疗。方法：回顾性分析我院收治的1例特发性肺含铁血黄素沉着症患儿的临床资料及影像学检查等。结果：该病临床症状不典型,病初易误诊,但临床医师若加强对该疾病认识,仔细分析病史,及时完善胸部影像学及相关检验项目可辅助诊断,降低误诊率。结论：特发性肺含铁血黄素沉着症可通过各项临床症状、胸部影像学,尤其是空腹胃液、纤维支气管镜肺泡灌洗液找到肺含铁血黄素巨噬细胞确诊。

The Analysis of Misdiagnosis to a Case of Idiopathic Pulmonary Hemosiderosis

Objective： To study the clinical diagnosis,misdiagnosis and therapies of idiopathic pulmonary hemosiderosis. Methods：The clinical data and imaging findings of a case of idiopathic pulmonary hemosiderosis were retrospectively analyzed. Results： The clinical symptoms were too atypical to be misdiagnosed. But if clinicians enhanced the understanding of the disease,analyzed clinical manifestation carefully,and completed chest images and inspection items in time,the misdiagnosis rate would be reduced. Conclusions：Analyzing the clinical manifestation,chest imaging findings,particularly the presence of hemosiderin macrophagocyte in bronchoalveolar lavage fluid,and gastric juice or lung tissue may provide a confirmed diagnosis of idiopathic pulmonary hemosiderosis.
