小细胞及非小细胞肺癌CD+4细胞因子水平的观察

 目的 观察Ⅲb期及Ⅳ期小细胞肺癌（SCLC）及非小细胞肺癌（NSCLC）患者辅助性T淋巴细胞亚群（Th1、Th2）细胞因子表达的特点，探讨不同病理类型肺癌的免疫学特点。方法 选择TNM分期为Ⅲb期和Ⅳ期的肺癌患者，分别按照SCLC、NSCLC分为两组，采取静脉血标本，用Th1、Th2细胞因子标记流式细胞技术检测干扰素（IFN-γ）、肿瘤坏死因子（TNF-α）、白细胞介素（IL）-2、-4、-6、-10，比较相同分期的SCLC与NSCLC患者外周血CD+4细胞分泌细胞因子水平的变化。结果 Ⅲb期SCLC患者周围血CD+4细胞表达TNF-α明显高于NSCLC患者（10.57±2.94）%、（7.03±3.06）%]（P＜0.05），而IL-4明显低于NSCLC患者（2.48±0.55）%、（4.32±1.74）%]，Ⅳ期SCLC患者外周血CD+4细胞表达Th1／Th2（IFN-γ／IL-4）低于Ⅳ期NSCLC患者。结论 SCLC中Th2细胞因子的高表达及IFN-γ／IL-4的降低可能预示预后不良，在NSCLC中，TNF-α的增高对预后更加有意义。

Investigation of CD4+ T cell cytokine of SCLC and NSCLC

Objective To explore the feature of Th1, Th2 cytokine in SCLC and NSCLC of stage Ⅲb or stage Ⅳ and the immuno-characteristic of different pathological type of lung cancer. Methods IFN-γ, TNF-α, IL-2, IL-4, IL-6 and IL-10 in venous blood from lung cancer patients of stage Ⅲb and stage Ⅳ were detected by flow cytometry marked by Th1 and Th2 cytokines, and then compared the difference of CD+4 cytokine of SCLC and NSCLC in same stage. Results In stage Ⅲb, TNF-α of SCLC was much higher than NSCLC(10.57±2.94)%, (7.03±3.06)%] (P＜0.05), and IL-4 was obviously lower than that of NSCLC(2.48±0.55)%, (4.32±1.74)%]. In stage Ⅳ, Th1/Th2（IFN-γ/ IL-4）of SCLC was significantly lower than that of NSCLC(P <0.05). Conclusion The expression of Th2 cytokine and depression of Th1/Th2（IFN-γ/ IL-4 ）in SCLC may hint prognosis mala. In NSCLC high expression of TNF-α may have more significance.
