超声引导下Mammotome微创旋切术在乳腺肿瘤中的应用

摘    要:目的探讨超声引导下Mammotome微创旋切术在乳腺肿瘤中的应用价值。方法对196例285个乳腺肿瘤进行超声引导下Mammotome微创旋切术。结果 285个乳腺肿瘤均被超声引导下Mammotome微创旋切术完全切除,每个肿瘤平均旋切13次(5~28次),手术时间30min(15~60min)。2例患者术后并发皮下血肿,余无并发症。手术瘢痕长2.5~5.0mm且较隐蔽。术中冰冻切片报告乳腺纤维瘤175例,乳腺腺病12例,导管内乳头状瘤6例,导管内原位癌3例。196例均应用超声检查进行术后随访,均未提示局部复发。结论应用Mammotome微创旋切系统切除适当大小乳腺肿瘤,可完全切除肿瘤并明显减少术后瘢痕,达到美容美观效果,并发症少,且可早期诊断可疑恶性肿瘤。

Abstract:Objective To investigate the value of ultrasound-guided minimally invasive Mammotome biopsy system in the treatment of breast tumor. Methods Totally 196 cases in 285 breast lesions were treated by uhrasound-guided minimally invasive Mammotome biopsy system. Results The 285 lesions in the 196 patients were excised completely by using the Mammotome system. The excision was performed for a mean of 13 times in each case ( ranged from 5 to 28 times) with mean operation time of 30 minutes ( 15 to 60 minutes). Skin ecchymosis was found in 2 patients after operation. The length of the incisional scar was not obvious with a range from 2.5 to 5.0 ram. Of all, 175 lesions were diagnosed as fibroadenoma, 12 lesions as breast adenosis, 6 lesions as intraductal papilloma, 3 lesions as breast cancer. The 196 cases were followed up by ultrasound and no local recurrence was found. Conclusions Ultrasound-guided minimally invasive Mammotome biopsy system is feasible for total resection of breast tumor with appropriate sizes. The technique leads to rare complications and good cosmetic results, which may provide an option for diagnosis and treatment of suspicious breast lesions.
