交锁髓内钉手术失败的原因分析

目的:分析交锁髓内钉失败的病例,总结经验教训。方法:2003年7月~2006年3月共对87例患者因四肢长管骨骨折进行开放复位,交锁髓内钉内固定,并对其中手术失败的原因进行分析。结果:术后总结在同一失败病例中,失败的原因常常是多种原因并存,常见的原因有骨延迟愈合和骨不连共9例,断钉4例,锁钉脱出8例,术后感染1例,再骨折1例。结论:交锁髓内钉是一种有效的骨折内固定治疗方法,其长期可靠的疗效与严格掌握手术指征,髓内钉的适当选择和手术中正确操作有关。

Analysis of failed interlocking intramedullary nailing

Objective:To discuss the failed interlocking intramedullary nailing and to sum up our experience and lesson. Metheds: 87 patients with various limb fractures were treated by open reduction and internal fixation with interlocking intramedullary nailing from July 2003 to March 2006. The cause of failed interlocking intramedullary nailing was analysed. Results: The result was evaluated not to show the factors related to the failures after the postoperative.The most common causes found in 9 cases were non union or delayed union,breakage were seen in 4 cases, screw backing out in 8 cases, infection in 1 case,refracture in 1 case. Conclusion: Interlocking intramedullary nailing could provide an effective treatment of the internal fixation.Long-term excellent clinical results were related to strict selection of indications,appropriat interlocking intramedullary nailing, and finally the experience and surgical technique of thesurgeon were also of importance to the final outcome.
