经导管封堵器介入治疗继发孔型房间隔缺损的临床研究

目的评价经导管封堵器治疗继发孔型房间隔缺损(ASD)的临床疗效及安全性.方法42例继发孔型房间隔缺损患者在X线透视及经胸超声心动图(TTE)监视下经导管置入封堵器封堵ASD,术后72 h、1个月、3个月分别行经胸超声心动图(TTE)评价治疗效果.结果除2例因房间隔缺损太大(大于40 mm)外,全组患者封堵器植入均获成功,技术成功率100%,选用的封堵器直径8～40 mm,术中无任何严重并发症,全部封堵病例无残余分流.3个月随访,所有病人临床症状及心脏大小改善,右室收缩压明显下降.结论经导管封堵器治疗ASD是一种有效、安全的介入治疗方法.

The clinical study of transcatheter closure of secundum atrial septal defect with occluder device

Aim To evaluate the efficacy and safety of transcatheter closure of secundum atrial septal defect(ASD) with occluder device. Methods Fourty-two patients with secundum ASD were treated using occluder device through the percutaneous procedure under flouroscopy and transthoracic echocardiography (TTE) guidance. TTE was performed 72 hours, 1 month, 3 months respectively after the procedure to evaluate the efficiency. Results The diameter of the occluder selected was from 8 mm to 40 mm. The success rate of technique was 100%, except the size of ASD was too large to be occluded in two cases, and no complications occured during procedure. No residual shunts were found. There was improvement in the symptoms and in cardiac size, and right ventricle systolic pressure declined significantly in all patients after 3 months follow-up. Conclusion Transcatheter closure using occluder device is an efficient and safe interventional procedure for patients with ASD.
