舌针配合语言训练治疗脑梗塞致运动性失语的临床研究

目的：观察针刺配合语言训练治疗缺血性中风失语症的临床疗效,探讨针刺治疗本病的机理。方法：两组共60例病人纳入研究,以随机数字表法将合格的研究对象分为试验组与对照组,试验组为针刺配合语言训练治疗,对照组为单纯语言训练。针刺治疗以舌针取穴为主;语言训练分口腔发音器官的训练,口形及声音训练,口语表达训练以及实用交流能力的训练。在疗程前后以《汉语标准失语症检查表》（ABC法）各作一次语言检测评价;以及疗程前后进行事件相关电位（P300）检测。结果：两组疗效有差异,其中治疗组疗效比对照组疗效好（P〈0.05）,治疗组（ABC）汉语失语检查法各亚项（信息量,流利性,复述,词命名,颜色命名,反应命名,是否,听辨认,执行指令）分值明显高于对照组,两组比较有显著性差异（P〈0.05）,治疗1个疗程后,治疗组P300电位成分中N2,P3波潜伏期缩短,和P3波幅升高明显,与对照组比较有显著性差异（P〈0.05）。结论：针刺配合语言训练治疗脑梗塞致运动性失语疗效显著,其效果优于单纯的语言训练组。汉语失语检查法各亚项（信息量,流利性,复述,词命名,颜色命名,反应命名,是否,听辨认,执行指令）分值提高,P300电位成分中N2,P3波潜伏期缩短,和P3波幅升高,可作为临床评价和预测针刺治疗缺血性中风运动性失语症的疗效指标,也是临床治疗机理之一。

Clinical Research on Broca Aphasia from Ischemic Apoplexy Treated by Acupuncture Associtated with Language Training

Objective：To observe the therapeutic effect of aphasia from ischemic apoplexy treated by acupuncture asso-ciated with language training.And discuss treatment mechanism of acupuncture for the disease.Metheds：There were sixty patients with broca aphasia from ischemic apoplexty involved in the trial.All patients completed whole study process without missing or eliminated and were randomly divided into the treatment group（acupuncture associated with language training） and the control group（language training only）.Acupuncture treatment mainly includes tongue-acupuncture therapy and language training includes the training of organ of speech,mouth-shape and voice,spoken language expression and practical exchange ability.Two groups were given language examination evaluation according to Aphasia Battery of Chinese（ABC） and accident correlated electric potential（P300） examination before and after the period of treatment.Results： All patients were uniformly divided into two groups and every group completed the trial well.The conditions of two groups were comparable（P 〉 0.05）.The trial showed that there were obvious differences between the control group and treatment group.The treatment effect of treatment group was better than that of the control group（P 〈 0.05）.According to the evaluation of all items of ABC（information measure,fluent character,repeat speak,vocabulary denominate,color denominate,response denominate,whether or not,hear dialectics,carry out instruction）,the score of treatment group was remarkably higher than that of control group.There were obvious differences between the control group and treatment group（P 〈 0.05）.After a period of treatment the incubation period N2,P3 of accident correlated electric potential（P300） had been markedly shortened（P 〈 0.05）,and the amplitude P3 had been obviously raised.The treatment effect of treatment group was remarkable difference compared to that of control group（P 〈 0.05）.Conclusion： The treatment effect of broca aphasia from apoplexy treated by acupuncture associated with language training was remarkable.Obviously the treatment group was superior to the control group（language training only）.The score of all items of ABC（information measure,fluent character,repeat speak,vocabulary denominate,color denominate,response denominate,whether or not,hear dialectics,carry out instruction） had been increased.The incubation period N2,P3 of accident correlated electric potential（P300） had been shortened（P 〈 0.05）,and the amplitude P3 had been raised.It could be regarded as one of indexes of appraising and predicting acupuncture clinical curative effect and one of therapeutic mechanism as well
