老年髋关节结核的诊治体会

目的提高对老年髋关节结核的认知意识和诊治水准，防止误诊误治的发生。方法对2001年1月～2007年8月收治的11例老年髋关节结核患者的诊治经过进行回顾性分析。男6例，女5例；年龄55～78岁，平均67岁；左侧7例，右侧4例。总结其发病特点及诊治体会。结果11例患者入院时均误诊为其他疾病，5例行髋关节病灶清除术，2例行髋关节翻修术，1例行内固定取出、病灶清除术，3例口服抗结核药物，均治愈。结论老年髋关节结核因解剖及年龄特点容易出现误诊、误治，提高对该病的认知意识，了解其发病特点，做好鉴别诊断是防止其误诊、误治的有力措施。

The diagnosis and treatment of senile hip joint tuberculosis

Objective To enhance the recognition of senile hip joint tuberculosis,and prevent misdiagnosis and mistreatment.Methods Eleven old patients who suffered from hip joint tuberculosis were admitted from January,2001 to August,2007.A comprehensive retrospective review of the process of treatment was carried out to analyze the characteristics of pathogenesis and experience of diagnosis and treatment.Results All these patients were misdiagnosed when admitted.Five of them underwent focal debridement;two underwent the revision of the hip;And one underwent removal of internal fixation and focal debridement,Three received oral anti-tuberculosis medicine.All patients were cured.Conclusions Due to its anatomical and aging characteristics,hip joint tuberculosis in senile patients can be easily misdiagnosed and mistreated.Enhancing awareness,understanding the pathogenic characteristics and making proper differential diagnosis are effective ways to avoid misdiagnosis and mistreatment.
