“三阶梯疗法”治疗膝关节骨性关节炎150例

目的 :观察三阶梯疗法治疗膝关节骨性关节炎的临床疗效。方法 :选取2017年1月至7月于山东省医学科学院颈肩腰腿痛医院就诊的膝关节骨性关节炎患者150例,参照膝关节骨性关节炎临床分期标准分为早期(47例)、中期(52例)、晚期(51例)三个治疗组。按照三阶梯疗法,早期组给予定点平衡力学手法治疗,中期组给予定点平衡力学手法+内服、外用创新型中药制剂治疗,晚期组给予定点平衡力学手法+内服、外用创新型中药制剂+银质针刀松解配合关节腔内药物注射治疗。治疗3个疗程后,观察三组患者的膝关节WOMAC评分及疼痛视觉模拟评分(VAS)变化,评价三阶梯疗法的临床疗效。结果:三组患者治疗后膝关节WOMAC评分及VAS评分较治疗前均降低,差异具有统计学意义(P0.05)。早、中、晚期三组的治疗总有效率分别为97.87%、92.31%、80.39%。结论:三阶梯疗法治疗膝关节骨性关节炎临床疗效显著,且治疗越早,疗效越好。

Three Ladders Therapy For Treating 150 Cases of Knee Osteoarthritis

Objective:To observe the clinical effect of three ladders therapy on treatment of knee osteoarthritis( KOA). Methods : A total of 150 patients with KOA who visited the Hospital for Neck and Shoulder Pain of Shandong Academy of Medical Sciences from January,2017 to July,2017 were selected as the study objects. According to the clinical criteria of KOA,the patients were divided into early-stage group(47 cases),mid-stage group(52 cases) and advanced-stage group(51 cases). With the three ladders therapy,patients in the early-stage group were treated with fixed-point equilibrium mechanics manipulation. Patients in the mid-stage group were treated with fixed-point equilibrium mechanics manipulation+ internal and external use of innovative traditional Chinese medicine. Patients in the advanced-stage group were treated with fixed-point equilibrium mechanics manipulation+ internal and external use of innovative traditional Chinese medicine+ silver needle knife release and intra-articular drug injection. After 3 courses of treatment,changes of knee joint WOMAC score and visual analogue scale(VAS) pain score of the three groups were observed to evaluate the clinical effect of the three ladders therapy. Results:After treatment,the WOMAC scores and VAS pain scores of the three groups were lower than those before treatment and the differences were statist ically significant(P<0.05). The total effective rate of the early-stage group,mid-stage group and advanced-stage group was 97.87%,92.31%,80.39%. Conclusion:The three ladders therapy is obviously effect for the treatment of KOA and earlier treatment can get better curative effect.
