电视腹腔镜在外伤性血气胸的诊断及治疗中的作用探讨

目的探讨电视腹腔镜在外伤性血气胸诊断及治疗中的作用。方法选择我院胸外科收治的76例外伤性血气胸患者,随机平均分为两组,研究组(38例)采用电视腹腔镜治疗,对照组(38例)采用传统开胸治疗,对比两种手术方式对患者创伤的大小,同时对比手术时间、术中出血量、术后疼痛、胸导管引流量、住院时间以及术后并发症发生情况。结果研究组术后第1、3、5天CRP浓度显著低于对照组,统计学有显著性差异(P<0.05)。研究组手术时间、住院时间显著短于对照组;术中出血量、术后镇痛药物用量、胸导管引流量显著少于对照组,P<0.05。两组术后并发症发生率相当(P>0.05)。结论电视腹腔镜治疗外伤性血气胸能达到传统开胸手术同样的诊疗效果,创伤小、出血少、术后恢复快。

Effect of TV laparoscopy in the diagnosis and treatment of traumatic hemopneumothorax

Objective To discuss the effect of TV laparoscopy in the diagnosis and treatment of traumatic hemopneumothorax. Methods 76 patients with traumatic hemopneumothorax were randomly and evenly divided into two groups. The control group were given the traditional thoracotomy treatment, and the study group were treated with TV laparoscopy. The trauma, duration of operation and hospi- tal stay, intraoperative blood loss, postoperative pain, now of thoracic duct, and postoperative complications were observed. Results The level of C reactive protein was lower in the study group than in the control group with significant difference after the treatment （P 〈 0.05 ）. The duration of hospital stay and operation was significantly shorter in the study group than in the control group （ P 〈 0. 05 ）. The intraop- erative blood loss, postoperative analgesia drug dosage and thoracic duct flow were significantly lower in the study group than in the control group （ P 〈 0. 05 ）. There was no no significant difference in postoperative complications between the two groups （ P 〉 0. 05 ）. Conclusion Compared with traditional thoracotomy treatment, TV laparoseopy has the same clinical curative effect and the advantages of mini trau- ma, less blood loss and lower rate of adverse effects, which is worth to be promoted in clinical practice.
