胱抑素C对终末期肾病患者心血管事件的预测作用

摘    要:目的观察胱抑素C(Cys C)能否预测终末期肾病患者心血管事件的发生。方法收集终末期肾病患者67例,在其人选时测定Cys C、Scr、BUN、LDL-C、HDL-C、Hb、A1b和CRP,观察心血管事件发生情况。结果胱抑素C与血BUN(r=0.527,P<0.05),SCr(r=0.491,P<0.05)呈正相关。与年龄(r=0.326,P=0.05),eGFR(r=-0.499,P<0.005)呈负相关。对心血管事件的Kaplan-Meier生存分析提示高,Cys C水平组(≥3.14mg/L)比较低Cys C水平组(<3.14 mg/L)有更高的无事件生存率(P=0.028)。在单变量分析中,Cys C能有效预测心血管事件的发生(OR=2.622,95%CI 1.2415.518,P=0.010)。结论血清Cys C可能成为终末期肾病患者的心血管事件的独立标志物。可能成为预测心血管事件的一个新角色。

Abstract:Objective To investigate the role of serum cystatin C for prediction of cardiovascular events.Methods This study included 67 end stage renal disease patients.The following laboratory data were obtained;cystatin C,creatinine,blood urea nitrogen(BUN),lipid profile,hemoglobin,albumin and C reactive protein(CRP) levels were measured at the point of enrollment.Results Cystatin C and blood BUN(r=0.527,P 0.05),SCr(r=0.491,P 0.05) were positively correlated.With age(r=-0.326,P=0.05),eGFR(r=-0.499,P 0.005) were negatively correlated.Cardiovascular events,Kaplan Meier survival analysis showed that the Cys C levels(≥3.14 mg/L) group have a higher event-free survival rate than Cys C levels relatively low(3.14 mg/L) group(P=0.028).In univariate analysis,Cys C can effectively predict the occurrence of cardiovascular events(OR= 2.622,95%CI 1.241-5.518,P= 0.010).Conclusion Serum Cys C could become an independent marker for ESRD patients with cardiovascular events.It may become a new factor to predict cardiovascular events.
