两组不同抗生素组合的四联根除幽门螺杆菌疗效观察

目的　观察两组不同抗生素组合的四联治疗幽门螺杆菌 (HP)相关性胃炎和消化性溃疡的疗效。方法　将经胃镜、组织学检查和1 4 C—UBT或血清抗体检测证实有HP感染的胃炎和消化性溃疡患者 2 3 6例 ,随机分为A、A1、B、B1四组 ,A组 :用丽珠胃三联 ;A1组 :奥美啦唑 +丽珠胃三联 ;B组 :丽珠得乐 +瑞贝克 +呋喃唑酮 ;B1组 :B组药物 +奥美啦唑 ;四组均常规剂量 ,每日 2次 ,疗程 1周。对A1组和B1组中的 112例消化性溃疡病人在抗HP治疗 1周后继服奥美啦唑 3周。所有病人治疗结束 4周后 ,做胃镜、组织学检查和1 4 C—UBT观察HP根除率 ,了解溃疡愈合率。用药过程中同时观察药物副作用。结果　A1组 ( 91.9%)和B1组 ( 94.4%)的HP根除率高于A组 ( 83 .9%)和B组 ( 80 .6%) ,两组比较差异有显著性 (P <0 .0 5 )。A1组和B1组胃溃疡的愈合率分别为 73 .9%和 70 .8%,十二指肠球部溃疡的愈合率分别为 93 .9%和 96.9%,两组的十二指肠球部溃疡的愈合率明显高于胃溃疡 (P <0 .0 5 ) ,HP根除率差异无显著性 (P>0 .0 5 )。结论　丽珠胃三联 +奥美啦唑四联和奥美啦唑 +丽珠得乐 +瑞贝克 +呋喃唑酮四联的 1周疗法是根除HP和治疗消化性溃疡有效价廉的方案

The clinic study on the eardication of Helicobacter pylori with quadruple therapy by different antibiotics

Objective To compare the efficacy of the eradication Helicobacter pylori(H.pylori) with quadruple therapy by different antibiotics in the treatment of gastritis and Peptic ulcer with H.pylori infection.Methods 236 endoscopically and histologically or 14 C-UBT and serum antibody test proved H.pylori positive patients were divided into four groups randomly,GroupA:Livzon Weisanlian; Gruop A1:Omeprazole(PPI)+Livzon Weisanlian; Group B:LivonDe+Nol+Swibec+Farazolidone; Group B1:GroupB+PPI. All patients were given routine dose for 1 week;122 of peptic ulcer in group A and B had taken PPI for 3 weeks continully. At least four weeks after termination of treatment, Patients were evaluated by endoscopie and histologig and 14 C-UBT,The results of H.pylori eradication and status of ulcer healion as well as effect-side of antibiotics were calculated.Results The H.pylori eradication rates of A1(91.9%) and B1(94.4%) were significantly higher than those of group A(83.9%) and group B(80.6%) ( P <0.05); The healing rates of group A1(93.9%) and group B1 (96.9%) in duodeal ulcer case were significantly higher than those of group A1(73.9%) and group B1(70.8%) in gastric ulcer ( P <0.05); But The H.pylori eradication rates were nearly same between in A1 and B1( P >0.05).Conclusions Group A1 and B1 have been shown to be effective and cheap therapy for eradication of H.pylori and healing of peptic ulcer.
