腱鞘巨细胞瘤的影像学诊断(附13例分析)

目的：探讨腱鞘巨细胞瘤的影像学诊断。方法：回顾性分析13例经手术和病理证实的腱鞘巨细胞瘤的影像资料,局限型9例,弥漫型4例,所有病例均行X线检查,4例行CT平扫和增强扫描,9例行MR平扫,6例同时行增强扫描。结果：X线片仅显示关节旁软组织肿块和骨质侵蚀、破坏。CT平扫显示软组织肿块和骨质改变较X线片清晰,还有1例显示软组织钙化和液化,增强扫描软组织肿块呈轻到中度强化。MRI T1WI上7例呈较均匀等信号,2例呈不均匀等低信号;T2WI上3例呈不均匀等高信号,6例呈不均匀等低信号;6例增强扫描呈中度到明显不均匀强化。结论：X线和CT检查对大多数腱鞘巨细胞瘤可明确诊断;MRI检查、尤其是增强扫描对于发现特异性含铁血黄素信号和确定病变范围具有独特优势,有助于提高诊断符合率,同时对指导临床治疗和随访具有重要价值。

Imaging findings of giant cell tumor of tendon sheath(with report of 13 cases)
