C臂下三根外定位针小切口空心钉对股骨颈手术改良体会

目的：探讨C臂下三根外定位针结合小切口空心钉对治疗股骨颈骨折手术的改良疗效。方法：采取闭合复位确认骨折解剖复位或近似解剖复位，C臂机监护下三根外定位克氏针定位结合小切口空心钉治疗100例股骨颈骨折患者，术后观察。结果：术后随访1～3年，骨折一般于4～6周愈合，100例患者均愈合良好，髋关节活动功能恢复好，无并发症。功能评价：优85例（85％），良13例（13％），可2例（2％），无股骨头坏死、不愈合、髋内翻等异变。结论：能通过手法牵引闭合复位好的股骨颈骨折应尽量选择损伤小的手术方法，采用三根外定位克氏针与空心钉联合治疗法，在切口小、手术时间短、出血量少、C臂透视时间短、预防并发症等方面有显著疗效及优势。

Under the C Arm Three Positioning Needle in Combination with Small Incision for the Femoral Neck Surgery Improved Experience

Objective：To explore the outside C arm under the first three positioning for closed reduction and hollow screw nails improved curative effect for the treatment of femoral neck fracture surgery. Methods： Adopt closed reduction and confirmed in accordance with the three hollow nail treatment of femoral neck fracture surgery indications, C arm machine custody three root position gram needle positioning and 100 patients with femoral neck fracture cannulated screw combination therapy, postoperative observation. Results： Postoperative follow-up of 1--3 years, fracture generally in 3~6 weeks to heal, 100 patients were all healed well, the hipjoint function recovery activities well, no complications. Function assessment：85 cases （85%）, 13 cases （13%）, can be in 2 cases （2%）,no necrosis of femoral head, don＇t heal, hipvarus and other abnormalities. Conclusion：Through closed traction reduction should try to choose good femoral neck fracture damage small surgical methods, especially the three outside the needle was positioning and cannulated screw combination therapy, in small incision, shorter operation time, less blood loss, short C arm fluoroscopy time, prevent complications and has significant curative effect and advantage.
