全麻复合硬膜外阻滞对布加氏综合征手术患者内分泌的影响

目的观察胸段硬膜外阻滞对布加氏综合征患者血糖、胰岛素和皮质醇水平的影响.方法46例择期布加氏综合征患者随机分成两组:Ⅰ组单纯全麻组;Ⅱ组全麻复合硬膜外组.分别与麻醉前、气管插管后、术中探查、术毕、拔管抽取静脉血测定血中血糖、胰岛素和皮质醇的值.结果Ⅰ组术中血糖、皮质醇测定值与麻醉前相比明显升高(P＜0.05),胰岛素较麻醉前升高但没有统计学意义.Ⅱ组麻醉前后测定值无明显变化.结论全麻复合硬膜外阻滞在布加氏综合征手术时内分泌影响小,是一种理想的麻醉方法.

Impact of epidural block combined general anesthesia on endocrine in patients with Budd-Chinri syndrome

Objeetive: To observe the impact on blood glucose,insulin and corticosteroid inpatients with Budd-Chiari syndrome undergoing thoracic epidural block anesthesia. Methods: 46 cases of Budd-Chiari syndrome were randomly divided into two groups: group I of single general anesthesia and group II of general anesthesia combined epidural block. Blood glucose, insulin and corticosteroid were tested at pre-anesthesia,post-endotracheal intubafion, operating, post-operating and pre-removing intubation. Results: The blood glucose and corticosteroid during operating were significantly higher than that before anesthesia (P <0.05) in the group, the insulin level was too higher during operating but there was no statistic significance. These values had no evident changes surrounding anesthesia in group II. Conclusion: General anesthesia combined epidural block can be regarded as an ideal anesthesia method with less impact on endocrine of patients with Budd-Chiari syndrome.
