根除幽门螺杆菌治疗对消化性溃疡患者血清胃泌素及胃泌素基因表达水平的影响研究

目的：探讨消化性溃疡的患者幽门螺杆菌根除治疗对其血清胃泌素和胃泌素基因表达的影响。方法：选取我院消化科消化性溃疡患者96例,行幽门螺杆菌根除治疗,前两周给予质子泵抑制剂（洛赛克）、甲硝唑和阿莫西林联合治疗,2周后继续服用胃舒散4周。治疗前及治疗结束1个月后分别应用放射免疫法测定所有患者的血清胃泌素水平和胃泌素基因的表达情况的变化。结果：观察组Hp根除治疗患者在治疗前胃泌素的基因表达水平为0.41±0.12μg/gpro,治疗后降为0.25±0.09μg/gpro;观察组十二指肠溃疡Hp根除患者治疗前平均胃泌素水平为39.6±11pmoL/L,治疗后1个月降为28.0±9.5pmoL/L,前后差异明显,有统计学意义（P〈0.05）;但胃溃疡患者治疗前后胃泌素水平无明显改变。结论：幽门螺杆菌感染可致患者胃泌素基因表达及分泌水平发生改变,根除幽门螺杆菌治疗可使胃泌素基因表达水平下降,胃泌素分泌减少。

Impact Study of Helicobacter Pylori Eradication Therapy on Expression Level of Serum Gastrin and Gastrin Gene of Patients with Peptic Ulcer

Objective： To observe the influence of helicobacter pylori eradication therapy on expression of serum gastrin and gastrin gene patients with peptic ulcer.Method： 96 patients with peptic ulcer in our hospital were selected and given helicobacter pylori eradication therapy.The patients were treated with proton pump inhibitor（losec）,metronidazole combined with amoxicillin in the first two weeks.After 2 weeks,the patients continued to apply weishu powder for 4 weeks.Applying radioimmunoassay to determine the serum gastrin level and gastrin gene expression condition respectively one month before the treatment and one month after the treatment.Result： As for the patients with Hp eradication therapy in the observation group,the gastrin gene expression level before treatment was 0.41±0.12μg/gpro,and it decreased 0.25±0.09μg/gpro after treatment.The average levels of gastrin of duodenal ulcer of the patients with Hp eradication therapy in the observation groupin the observation group was 39.6±11pmoL/L,and it decreases to 28.0±9.5pmoL/L one month after treatment.The difference was evident and had statistical significance（P〈0.05）.But as for gastrin level of the patients with gastric ulcer,there was no great difference before and after the treatment.Conclusion： Helicobacter pylori infection can cause gastrin gene expression and secretion to change.Helicobacter pylori eradication therapy could make gastrin gene expression level drop and gastrin secretion reduce.
