大剂量洛赛克联用善宁治疗难治性上消化道出血

目的]探讨大剂量洛赛克联用善宁治疗难治性上消化道出血的疗效。方法]111例患者随机分为治疗组(56例)和对照组(55例)。2组均用善宁0.1mg静脉注射后,以25μg/h微量泵持续静脉推注,出血停止后维持48h。治疗组同时应用洛赛克80mg静脉缓慢推注,继之以8mg/h微量泵持续静脉推注;对照组同时应用洛赛克40mg静脉推注,继之以4mg/h微量泵持续静脉推注。2组均于出血停止后维持48h。结果]治疗组显效率及总有效率均高于对照组(均P<0.05);2组急性静脉曲张性上消化道出血疗效差异无统计学意义,急性非静脉曲张性上消化道出血总有效率差异有统计学意义(P<0.01)。结论]大剂量洛赛克联用善宁治疗难治性上消化道出血疗效优于常规剂量。

High dose losec combined with sandostatin treatment for medically refractory upper gastrointestinal bleeding

Objective To explore the efficacy and the safety of high dose of losec combined with sandostatin treatment for medically refractory upper gastrointestinal bleeding.MethodsOne hundred and eleven patients were divided into high dose group(56cases)and control group(55cases)with stratified randomization All the cases of two groups were treated with sandostatin 0.1 mg iv and then 25ug/h continuous iv with micropump up to 48 h later of hemostasis.The patients in high dose group received losec 80 mg iv and then 8 mg/h continuous iv with micropump in the same time.The patients in control group received losec 40 mg iv and then 4 mg/h continuous iv with micropump in the same time.Two groups were used up to 48h later of hemostasis.Results The effective rate and total response rate were significant statistically between the two groups(P<0.05).The efficacy of the cases with acute variceal upper gastrointestinal bleeding (AVUGIB) was no significant statistically between two groups(P>0.05).The total response rate of the cases with acute nonvariceal upper gastrointestinal bleeding(ANVUGIB) was significantly different(P<0.01)between two groups.The bleeding volume from treatment start to end and the mean hemostasis time were both significantly different between two groups(P<0.01).Except two cases with spirit symptom were found in both groups no severe side-effect was found else.ConclusionThe efficacy that high dose losec combined with sandostatin treatment for medically refractory upper gastrointestinal bleeding is more excellent than the routine dose losec.The adverse reaction does not increase.
