探讨了红外热像图对强直性脊柱炎(AS)的诊断价值.采用国产WP-95型医用红外热像扫描仪,对131例AS患者和100例正常人的红外热像图表现进行分析.结果表明,AS的脊背热像图自颈胸至腰骶呈温高色红的热区,宽度相当于两侧骶棘肌外缘间,平均1.7℃的连线,温度高于两侧温度1~2.9 ℃.可见用热像图诊断AS具有明显的临床意义.  相似文献

目的:探索强直性脊柱炎理法方药相应规律.方法:在CBM数据库中收集治疗强直性脊柱炎文献数据,采用基于敏感关键词频数统计的数据分层算法,挖掘强直性脊柱炎的相关规律,并通过一维频次的解释及二维的网络图进行展示.结果:强直性脊柱炎证侯方面以肾阴虚、肝肾阴虚为主;治法以温补、通络、化瘀为主;方药更多的是补肝肾,益气血、活血通络药物,温经散寒之品.结论:文本挖掘技术,结合文献回溯,人工阅读降噪,能够比较客观的总结中医“证-症-方-药”的规律,为中医客观化,以及中医临床提供有益的探索与参考.  相似文献

目的探讨强直性脊柱炎重度后凸矫形的围手术期护理。方法对24例强直性脊柱炎重度后凸畸形患者,施行单纯后路蛋壳截骨后凸矫形术,术前进行心理护理、营养支持、呼吸功能训练等护理干预,术后监测生命体征、观察脊髓神经功能、防止各种并发症和加强康复训练。结果术后患者达到70%的矫形效果,肺活量明显增加,未出现严重并发症。结论有效的围手术期护理有助于降低重度脊柱后凸矫形术后并发症的发生,提高临床治疗效果。  相似文献

目的探讨女性强直性脊柱炎的诊断特点。方法回顾性分析我院1999年1月-2009年1月诊治的93例女性强直性脊柱炎和191例男性强直性脊柱炎患者并进行临床特点比较。结果93例患者平均年龄35.05岁,平均病程7.48年,有生育史者68例(73.12%),HLA-B27阳性者59例(63.40%),主要临床特征以臀部、耻骨部、足跟部疼痛为主;影像学检查,单侧Ⅰ级39例,双侧Ⅰ级25例,单侧Ⅱ级15例,双侧Ⅱ级8例、Ⅲ级7例。结论女性强直性脊柱炎临床特点是病程长、症状轻,以外周关节疼痛为主,缺乏典型的影像学表现,致残率低,生育后病情可能加重。  相似文献

In a healthy body, homeostatic actions of osteoclasts and osteoblasts maintain the integrity of the skeletal system. When cellular activities of osteoclasts and osteoblasts become abnormal, pathological bone conditions, such as osteoporosis, can occur. Traditional imaging modalities, such as radiographs, are insensitive to the early cellular changes that precede gross pathological findings, often leading to delayed disease diagnoses and suboptimal therapeutic strategies. 18F-sodium fluoride (18F-NaF)-positron emission tomography (PET) is an emerging imaging modality with the potential for early diagnosis and monitoring of bone diseases through the detection of subtle metabolic changes. Specifically, the dissociated 18F- is incorporated into hydroxyapatite, and its uptake reflects osteoblastic activity and bone perfusion, allowing for the quantification of bone turnover. While 18F-NaF-PET has traditionally been used to detect metastatic bone disease, recent literature corroborates the use of 18F-NaF-PET in benign osseous conditions as well. In this review, we discuss the cellular mechanisms of 18F-NaF-PET and examine recent findings on its clinical application in diverse metabolic, autoimmune, and osteogenic bone disorders.  相似文献

To examine serum interleukin 18 (IL-18), fetuin-A, soluble intercellular adhesion molecule-1 (sICAM-1), and endothelin-1 (ET-1) levels in ankylosing spondylitis (AS), psoriatic arthritis (PsA), and Synovitis Acne Pustulosis Hyperostosis Osteitis syndrome (SAPHO). We studied 81 AS, 76 PsA, and 34 SAPHO patients. We measured serum IL-18, fetuin-A, sICAM-1, ET-1, IL-6, IL-23, vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF). IL-18 levels were higher in AS (p = 0.001), PsA (p = 0.0003), and SAPHO (p = 0.01) than in controls, and were positively correlated with CRP (p = 0.03), VEGF (p = 0.03), and total cholesterol (TC, p = 0.006) in AS and with IL-6 (p = 0.03) in PsA. Serum fetuin-A levels were lower in AS (p = 0.001) and PsA (p = 0.001) than in controls, and negatively correlated with C-reactive protein (CRP) in AS (p = 0.04) and SAPHO (p = 0.03). sICAM-1 positively correlated with CRP (p = 0.01), erythrocyte sedimentation rate (ESR, p = 0.01), and IL-6 (p = 0.008) in AS, and with IL-6 (p = 0.001) in SAPHO. Serum ET-1 levels were lower in AS (p = 0.0005) than in controls. ET-1 positively correlated with ESR (p = 0.04) and Disease Activity Score 28 (DAS28, p = 0.003) in PsA. In spondyloarthritis, markers of endothelial function correlated with disease activity and TC.  相似文献

Rheumatoid arthritis (RA), ankylosing spondylitis (AS), and psoriatic arthritis (PsA) are comprehensive immunological disorders. The treatment of these disorders is limited to ameliorating the symptoms and improving the quality of life of patients. In this study, serum samples from RA, AS, and PsA patients were analyzed with metabolomic tools employing the 1H NMR method in combination with univariate and multivariate analyses. The results obtained in this study showed that the changes in metabolites were the highest for AS > RA > PsA. The study demonstrated that the time until remission or until low disease activity is achieved is shortest (approximately three months) for AS, longer for RA and longest for PsA. The statistically common metabolite that was found to be negatively correlated with the healing processes of these disorders is ethanol, which may indicate the involvement of the gut microflora and/or the breakdown of malondialdehyde as a cell membrane lipid peroxide product.  相似文献

Human leukocytic antigen-B27 heavy chain (HLA-B27 HC) has the tendency to fold slowly, in turn gradually forming a homodimer, (B27-HC)2 via a disulfide linkage to activate killer cells and T-helper 17 cells and inducing endoplasmic reticulum (ER) stress to trigger the IL-23/IL-17 axis for pro-inflammatory reactions. All these consequences lead to the pathogenesis of ankylosing spondylitis (AS). Sulfasalazine (SSA) is a common medication used for treatment of patients with AS. However, the effects of SSA treatment on (B27-HC)2 formation and on suppression of IL-23/IL-17 axis of AS patients remain to be determined. In the current study, we examine the (B27-HC)2 of peripheral blood mononuclear cells (PBMC), the mean grade of sarcoiliitis and lumbar spine Bath Ankylosing Spondylitis Radiology Index (BASRI) scores of 23 AS patients. The results indicated that AS patients without (B27-HC)2 on PBMC showed the lower levels of mean grade of sarcoiliitis and the lumbar spine BASRI scores. In addition, after treatment with SSA for four months, the levels of (B27-HC)2 on PBMCs were significantly reduced. Cytokines mRNA levels, including TNFα, IL-17A, IL-17F and IFNγ, were also significantly down-regulated in PBMCs. However, SSA treatment did not affect the levels of IL-23 and IL-23R mRNAs.  相似文献
