Federal legislation regarding health care in the U.S. has increased rapidly in the past few years. A major law with potential far-reaching effects was enacted as a result of increasing legislation and rising health care costs. This law,The National Health Planning and Resources Development Act,has created a network of over 200 local, mostly nongovernment units, called health systems agencies. These agencies are responsible for areawide health planning, plan implementation, review and approval of federal health care expenditures for local programs, and facilities review. They will affect health and mental health programs at the local level. The article is directed to local health and mental health care providers who will, of necessity, deal directly with the local HSA's.  相似文献

This study of chronically mentally disabled persons in community residences examined the discriminant validity of subjective quality of life indicators and self-report mental health indices to determine the potential confounding effects of psychopathology on the assessment of quality of life (QOL). Factor analyses and difference-score reliabilities identified a general QOL construct and a general mental health construct with 27% common variance. However, psychiatric symptoms did not significantly (p less than .05) alter the bivariate and multivariate relationships among the QOL ratings, except in the health domain in which the correlations of health-related QOL indicators with global QOL were significantly (p less than .05) attenuated after removing the effects of psychopathology. The results suggest that psychopathology does not introduce bias into the overall structure of QOL data, but they also indicate the importance of controlling for mental health effects in the assessment of patients' self-rated health and satisfaction with health care.  相似文献

Population-to-practitioner ratios have long been the primary index in the designation of health manpower shortage areas. This paper documents that application of the widely used population-to-dentist index results in understatement of the need for dental health manpower in rural areas. Through the analysis of utilization data collected from a statewide health screening program in Colorado, the practice of sole reliance on the population-to-dentist indices as an indicator of need was tested. Another measure, the area-(square miles) to-dentist ratio was formulated, examined, and found to be a more useful referent of the need for additional health manpower in rural areas. Utilization of dental services in sparsely settled rural counties of Colorado was unrelated to population-to-dentist ratios. A strong, statistically significant association of utilization with land area-to-dentist ratios was found. The findings of this analysis suggest a need for reevaluation of needs assessment methodologies used in the designation of health manpower shortage areas. Indices more sensitive to consumer circumstance than to the number of health care providers available must be considered.  相似文献

We investigate institutional antecedents to subsidiary external embeddedness and relate regulation constraining competition in local service sectors to subsidiary embeddedness with local partners in complementary sectors. Combining research on business networks with arguments derived from transaction cost economics, we argue that subsidiary external embeddedness depends on the extent of transaction costs originating from small numbers bargaining, which regulatory competitive constraints in local service sectors are a source of. Based on this logic, we suggest that low and high levels of regulatory competitive constraints are associated with greater subsidiary external embeddedness. We also suggest that this U-shaped relationship is more pronounced for subsidiaries that are centers of excellence within the multinational enterprise because these subsidiaries heavily depend on the local context as a source of their competitive advantage over their sister subsidiaries.  相似文献

In Ethiopia, the study and issue of center–periphery relations is not an easy task. It has remained complex and dynamics as it has been dictated and shaped by ever changing socio-economic relations, state ideology and structure. A case in point is Metekel region, which had been characterized by under development, lowland, hot climate and traditional way of life. Until post-1991developments, Metekel was peripheral in relation to the central state. It was an area of confrontation and conflict. Owing to the structural weakness of the centre, successive rulers of the area did not command effective control over it. Focusing on Gumuz, a Nilo-Saharan family, on one hand, and highlanders and new settlers, on the other, this essay examined the main features and dynamics of center–periphery relations, governance and conflicts in Metekel area. Concurrently administered personal and telephone surveys are compared to measure differences between the modes for identical questions. Speed of questioning is found to be greater in telephone interviews than personal interviews. The faster pace of telephone interviews is linked to shorter answers to open-ended items on the telephone. The tendency to reduce such responses is disproportionately exhibited by younger, affluent respondents who tend to provide detailed responses in personal interviews.  相似文献

Social Science researchers have advanced important yet somewhat contradictory conclusions regarding the different economic and occupational reward structures faced by men and women. Income and wage differences between men and women have been shown to be sizable and persistent throughout the occupational hierarchy. Conversely, gender differences in occupational status, commonly scaled by the Duncan Socioeconomic Index, have been shown to be small or nonexistent in most studies. In an attempt to investigate this incongruity, the present study undertakes a comparison of the Duncan SEI and the Nam-Powers Occupational Status Scores in an empirical study of the occupational position of white men and women in 65 large standard metropolitan statistical areas (SMSAs) in 1970. While the findings of earlier studies showing no gender differences in occupational status are for the most part replicated using the Duncan SEI, use of the Nam-Powers scores prompts a much different conclusion. Large status differences between men and women are indicated using this latter scale, differences which are very much in line with income differences commonly cited. We suggest that the Nam-Powers metric should be used instead of the Duncan SEI in studies of occupational status of women and men.  相似文献
