心源性猝死一级预防植入心律转复除颤器前非持续室性心动过速与植入后恰当治疗的关系

目的探讨植入型心律转复除颤器(implantable cardioverter defibrillator,ICD)用于心源性猝死一级预防时,植入前发生非持续性室性心动过速(non-sustained ventricular tachycardia,NSVT)与植入后ICD恰当治疗的关系。方法本研究为单中心回顾性队列研究,连续纳入2006年1月至2017年12月在北京协和医院住院植入ICD用于一级预防的心脏病患者,患者在住院期间ICD植入前均完善Holter检查。根据Holter检查是否出现NSVT,将患者分为NSVT组及无NSVT组。门诊或电话随访至2018年8月,随访事件包括全因死亡、植入后ICD放电治疗及抗心动过速起搏(antitachycardia pacing,ATP)治疗情况。结果纳入60例符合入选和排除标准的患者,随访时间37 (14～61)个月,KaplanMeier生存曲线及Log-Rank检验提示,NSVT组与无NSVT组相比,全因死亡率无统计学差异(P=0. 108),NSVT组恰当治疗的风险高于无NSVT组(P=0. 033),NSVT和左心室射血分数与ICD恰当治疗独立相关(NSVT:HR=5. 099,95%CI:1. 399～18. 588,P=0. 014;左心室射血分数:HR=1. 077,95%CI:1. 013～1. 145,P=0. 018)。结论 ICD一级预防患者植入前出现NSVT提示接受ICD恰当治疗的风险增加。

迪银片治疗银屑病临床疗效观察

目的观察迪银片治疗银屑病的临床疗效。方法30例银屑病患者,口服迪银片8周,采用PASI评分法评价治疗前后效果,并观察有无不良反应发生。结果患者痊愈率56.7%,显效率36.7%,总有效率93.3%,无明显的不良反应。结论迪银片是临床治疗银屑病的有效药物之一。     

Impact of a snow avalanche against an obstacle. Formation of shock waves

The paper is devoted to the effect of compressibility of the avalanche snow impacting an obstacle. Compression shocks generated by obstacle cause high pressure peaks at first instants of impact. That is why the account of compressibility is essential for the understanding of measurements and the design of structures. The main problem in calculation compression shocks in avalanches is to formulate an equation of state for moving snow in impact. Two different types of equations of state are proposed depending on the type of the avalanche (low-density and high-density flows). The approach is not totally new. It was earlier proposed mainly in Russian literature. Here a brief review of the previous work is given with discussion of some gaps in it. The theory is reformulated and further developed to account thermodynamical equations. The simplest case of a normal compression shock in an avalanche flow is studied. Examples of estimations of pressure and density behind a shock are given. It is important to emphasize that the Mach number plays an important role in the theory of compressible flows so it should be taken into account (together with the Froude number) in calculation and modelling an avalanche impact pressure.     

Impact of the addition of an enzyme pool on an activated sludge system treating dairy wastewater under fat shock loads

BACKGROUND: The application of lipase‐rich enzyme pools (such as the crude solid enzymatic preparation (SEP) obtained from Penicillium restrictum solid‐state fermentation of agro‐industrial wastes) to activated sludge systems may be an effective strategy for preventing various operational problems. The continuous addition of SEP to the treatment system can become cost‐prohibitive when in situ production and/or storage are factored in. The application of SEP to high‐fat wastewater treatment would only be justified as an emergency measure, such as a sudden increase in the fat content of the bioreactor influent. Therefore, the primary objective of this work was to investigate the efficiency of a crude SEP during fat shock loads, simulated through the periodic addition of dairy industry waste containing high fat concentrations to the feed stock of an activated sludge system, operated in continuous mode. RESULTS: The test bioreactor exhibited a higher average chemical oxygen demand (COD) removal efficiency than the control bioreactor (83% for control and 90% for test) and the fat accumulation in the biological flocs of the test bioreactor was 3.2 times lower than that in the control bioreactor. Turbidity was also lower in the effluent of the test bioreactor (123 and 66 FTU in control and test, respectively) and it had a shorter recovery time between shock loads, especially when the interval between loads was shorter than one month (biweekly and weekly shock loads). CONCLUSION: The addition of SEP during fat overloads in the reactor feed maintained efficient COD removal in the test bioreactor for 270 days without any operational problems. A place for Bobo in play therapy.

Some therapists hold that the Bobo and other aggressive toys should be excluded from the playroom as they elicit acting-out behavior that is transferred to the world outside the playroom. However, many therapists view the inclusion of these items as imperative elements that facilitate positive growth and maturation in the child. Interviews with authorities in the field of play therapy support the position that the inclusion of the Bobo is not only useful, but a therapeutic necessity. The book is divided into four parts. Part I examines the nature and function of emotions. Part II examines the first part of emotional coaching--arriving at emotion. Part III examines the second part of emotional coaching--leaving emotion. Finally, Part IV examines the application of emotion-focused therapy to the problems of living, in the context of individuals, couples therapy, and parenting. This book offers a wealth of therapeutic techniques and theoretical principles about the process of change. Healing the psyche through music, myth, and ritual.

According to Carl Jung, a person's symptoms occur as a result of the psyche's creative attempt to self-regulate. These symptoms are viewed as the symbol-making function of the psyche. Music therapy as a therapeutic tool is also viewed as having a symbol-making function in that the physical act of making music involves conceptualizing one's symptoms into sounds. The sound serves as a musical symbol of the symptom. The myth of Orpheus is offered as an example of how early man amplified the creative potential contained in music to heal from life's woes. The Orphic archetype illustrates that music serves as a medium for healing and as an expression of Self. That psyche can be healed by music is a reminder of the connection between music therapy and the myth of Orpheus. In using the therapeutic tool of music one can transcend, through the symbol making process inherent in music therapy, the tension of opposites that are created at the crossroads of disease and wellness. Ratings of Therapist Dynamic Activities and Alliance Early and Late in Psychotherapy.

The present study examines the concurrent and predictive relationships between therapist psychodynamic-interpersonal activity and therapist-rated alliance. Ratings from 45 patient and therapist dyads engaged in short term psychodynamic psychotherapy from a point early (3rd or 4th session) and late (the session at which 90% of the treatment was completed) in treatment were used. The results suggest that therapists who have positive views of the alliance early in treatment also have positive views of the alliance later in treatment. Therapists who used psychodynamic-interpersonal activities early in treatment also made use of psychodynamic-interpersonal activities later in treatment. Moreover, the use of psychodynamic-interpersonal activities early in treatment was positively related to both global and specific aspects of therapist-rated alliance late in treatment.
