We investigated hepatitis C virus (HCV) infection in 35 patients with hypertrophic cardiomyopathy and 40 patients with ischemic heart disease who were consecutively admitted to our hospital. Frequency of positive anti-HCV antibody was significantly higher in patients with hypertrophic cardiomyopathy (6 of 35 patients, 17.1%) than that in patients with ischemic heart disease (1 of 40 patients, 2.5%, p = 0.036). In three of these six patients with hypertrophic cardiomyopathy, HCV RNA was detected in myocardial tissue. In two of these three patients, HCV RNA was detected from biopsy and autopsy specimens of the ventricles, but not in the serum, suggesting that HCV may replicate in myocardial tissue and may be relevant to ventricular hypertrophy. Thus, HCV infection may play a role in the development of hypertrophic cardiomyopathy.  相似文献

A general topology optimization method, which is capable of simultaneous design of density and orientation of anisotropic material, is proposed by introducing orientation design variables in addition to the density design variable. In this work, the Cartesian components of the orientation vector are utilized as the orientation design variables. The proposed method supports continuous orientation design, which is out of the scope of discrete material optimization approaches, as well as design using discrete angle sets. The advantage of this approach is that vector element representation is less likely to fail into local optima because it depends less on designs of former steps, especially compared with using the angle as a design variable (Continuous Fiber Angle Optimization) by providing a flexible path from one angle to another with relaxation of orientation design space. An additional advantage is that it is compatible with various projection or filtering methods such as sensitivity filters and density filters because it is free from unphysical bound or discontinuity such as the one at θ = 2π and θ = 0 seen with direct angle representation. One complication of Cartesian component representation is the point‐wise quadratic bound of the design variables; that is, each pair of element values has to reside in a given circular bound. To overcome this issue, we propose an isoparametric projection method, which transforms box bounds into circular bounds by a coordinate transformation with isoparametric shape functions without having the singular point that is seen at the origin with polar coordinate representation. A new topology optimization method is built by taking advantage of the aforementioned features and modern topology optimization techniques. This paper deals with density-based topology optimization considering fluid and thermal interactions, in which the Navier–Stokes and heat transport equations are coupled. We particularly focus on designing heat exchangers. In the engineering context, heat exchangers are designed while considering a certain amount of input power. Therefore it is important to maximize the performance of a heat exchanger under a constant input power. In this paper we propose a way to control the input power by introducing an extra integral equation. To be more precise, in the fluid analysis, the inlet pressure is determined by solving the extra integral equation together with the Navier–Stokes equation. By doing this we can keep the inlet power constant even when the flow channels are changed in the optimization process. Consequently, the system of equations of the fluid field takes an integrodifferential form. On the other hand, in the heat transport analysis, a single governing equation is defined for simultaneously modeling both the solid and fluid parts. The design variable is a fluid fraction whose distribution represents the topology of the solid and fluid domains. When designing heat exchangers, two different heat conditions are considered in the formulation of the optimization problems, namely temperature-dependent and temperature-independent heat sources. Through the numerical examples for designing flow channels in a heat exchanger, it is shown that distinct topologies can be obtained according to the input power and the heat source conditions.  相似文献

In recent years, remarkable advancement of new power semiconductor devices, such as SiC and GaN, enables the increase of switching frequency of power converters, and hence the volume of passive components, such as ac filters and transformers, can be reduced. However, temperature rise caused by the inductor loss is increasing, and hence iron loss evaluation of the inductor is one of the most important issues to realize high power density converters. Conventionally, an improved generalized Steinmetz equation (iGSE) has proposed in order to calculate the iron loss under a pulse voltage magnetizing condition. However, accurate iron loss calculation of the ac filter inductor used in a PWM inverter cannot be realized. The authors have proposed two methods of iron loss evaluation of ac filter inductors. The first one is a loss map method which can calculate the iron loss without using a real PWM inverter. Another one is an ILA (Inductor Loss Analyzer) which can measure the iron loss in every switching period in a real PWM inverter. In this paper, comparisons of the iron loss between the ILA and the loss map method on both the single‐phase and three‐phase inverters are studied. It is found that iron loss of the ac filter inductor in the three‐phase PWM inverter which is calculated by the loss map method cause a large error on a specific condition. In order to prevent the calculation error, the authors proposed a revised loss map method and proved the effectiveness of the method.  相似文献

OBJECTIVES: This study was designed to examine the effects of denopamine, a selective beta1-adrenergic agonist, in a murine model of congestive heart failure (CHF) due to viral myocarditis. BACKGROUND: Positive inotropic agents are used to treat severe heart failure due to myocarditis. However, sympathomimetic agents have not been found beneficial in animal models of myocarditis. METHODS: In vitro: The effects of denopamine on lipopolysaccharide-induced tumor necrosis factor-alpha (TNF-alpha) production was studied in murine spleen cells. In vivo: Four-week-old DBA/2 mice were inoculated with the encephalomyocarditis virus (day 0). Denopamine (14 micromol/kg), denopamine (14 micromol/kg) with a selective beta1-blocker metoprolol (42 micromol/kg), or denopamine (14 micromol/kg) with metoprolol (84 micromol/kg) was given daily, and control mice received the vehicle only. Survival and myocardial histology on day 14 and TNF-alpha levels in the heart on day 6 were examined. RESULTS: In the in vitro study, TNF-alpha levels in treated cells were significantly lower than in controls (p < 0.05). In the in vivo study treatment with denopamine significantly improved the survival of the animals (14 of 25 (56%) treated, vs 5 of 25 (20%) control mice), attenuated myocardial lesions, and suppressed TNF-alpha production (66.5+/-7.5 pg/mg of heart in treated mice vs 113.5+/-15.1 pg/mg of heart in control mice, mean+/-SE). There was a strong linear relationship between mortality and TNF-alpha levels (r=0.98, n=4, p < 0.05). These in vitro and in vivo effects of denopamine were significantly inhibited by metoprolol. CONCLUSIONS: These results suggest that denopamine may exert its beneficial effects, in part, by suppressing the production of TNF-alpha via beta1-adrenoceptors.  相似文献

Our goal is the development of the morphological model faithfully representing the structure of a real vitrified-bonded wheel in order to determine the wheel structure to meet a required wheel grade and derive the wheel composition to form such a wheel structure. This paper describes the two and new morphological model for a vitrified-bonded wheel using fractal modeling techniques. One model is the “agglomerate model of a plastic mixture” representing a plastic mixture before the burning process of a wheel on the basis of the diffusion-limited aggregation algorithm and another is the “morphological model of a burnt wheel” representing the structure of the burnt wheel on the basis of the sandpile algorithm. Both models bear resemblances to the real productions in structure. In particular, the morphological model of a burnt wheel bears resemblances not only in morphology but also in bulk density to real wheel structure, and it provides the structural characteristics such as average grain spacing and number of bond bridges per grain that relate to the tensile strength of wheels. Therefore, our models play an important role in the development of new wheels.  相似文献

This paper deals with a design method for boundary shapes using filtering techniques based on a partial differential equation (PDE). In shape optimization, it is known that oscillatory boundaries appear when design variables are directly assigned to the design boundaries. In addition, during the optimization process, discretized elements in a computational domain are distorted due to extremely large shape changes along the design boundaries. The distorted elements may cause accuracy deterioration or numerical instability in a forward problem. In this paper, we propose a shape optimization method by using the PDE as a low pass filter which prevents the oscillatory boundaries of the optimized design. For restricting the shape distortion of the discretized elements, the shear deformation of the elements is constrained in the optimization problem. Mathematical programming is used to find the boundary shapes under the KKT conditions. The effectiveness of the proposed method is demonstrated through numerical examples in solid and fluid mechanics.  相似文献

Circulating levels of hepatocyte growth factor (HGF)/scatter factor have been recently found to be increased in the early phase of myocardial infarction, and it has been hypothesized that HGF plays a role in angiogenesis and collateral vessel growth. Heparin has also been shown to enhance angiogenesis and to improve collateral blood flow. This study was designed to study the effect of heparin on the release of HGF. In an experimental study, heparin was given to rats intravenously and plasma was collected for measurements of HGF by enzyme-linked immunosorbent assay. A dose-dependent increase in circulating HGF was measured with peak levels occurring 10 min after injection of 300 units/kg of heparin (15.4+/-2.0 ng/ml after v 0. 17+/-0.14 ng/ml before injection,P<0.0001). In a subsequent clinical study, 12 patients received 3000 units of heparin during cardiac catheterization. Circulating HGF increased steeply within 3 min of the injection. Comparable changes in plasma concentrations were measured in samples obtained from femoral vein (8.7+/-3.5 after v 0. 33+/-0.07 before injection P<0.05) or artery (10.5+/-3.2 ng/mlv 0. 27+/-0.05 P<0.01), pulmonary artery (9.1+/-2.0 ng/mlv 0.36+/-0.06 ng/ml,P=0.07 ) or right atrium (8.5+/-1.6 ng/mlv 0.42+/-0.11,P<0.01). This study suggests that heparin-induced effects such as the promotion of angiogenesis may be at least partly due to the release of HGF.  相似文献

We present our experience of transcutaneous truncal anaesthesia of the maxillary nerve in association with transmucosal anaesthesia of the sphenopalatine ganglion in surgically assisted rapid maxillary expansion. Twelve patients with a skeletal transverse discrepancy of the maxilla were treated in our department from 1994 to 1995. Maxillary transcutaneous nerve block was done with a Quincke 8 cm spinal needle together with transmucosal anaesthesia of the sphenopalatine ganglion. Mepivacaine without adrenaline and sodium bicarbonate 1/10 was used for truncal anaesthesia and lidocaine-prilocaine cream for transmucosal anaesthesia. A Le Fort I osteotomy, lateral nasal wall osteotomy, pterygomaxillary osteotomy, and a palatal osteotomy were done for all patients before the maxillary expansion. Total anaesthesia of the maxillary area facilitated the operations and appreciably reduced the amount of postoperative pain. The ease of achieving effective anaesthesia before and after operation and the absence of side-effects make this form of anaesthetic particularly useful in surgically assisted rapid maxillary expansion.  相似文献
