目的:探讨含重组人碱性成纤维细胞生长因子(recombinant human basic fibroblast growth factor,rhbFGF)和重组人骨形态发生蛋白-2(recombinant human bone morphogenetic protein-2,rhBMP-2)骨水泥在骨质疏松性腰椎压缩性骨折(osteoporotic vertebral compression fracture,OVCF)患者经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗的应用价值。方法:回顾性分析2018年1月至2021年1月收治的103例行PKP手术治疗的OVCF患者,男40例,女63例;年龄61~78(65.72±3.29)岁。受伤原因:滑倒33例,跌倒42例,提重物受伤28例。根据填充骨水泥不同分为3组:磷酸钙组34例,男14例,女20例,年龄(65.1±3.3)岁,填充磷酸钙骨水泥;rhBMP-2组34例,男12例,女22例,年龄(64.8±3.2)岁,填充含rhBMP-2的骨水泥;rhbFGF+rhBMP-2组35例,男14例,女21例,年龄(65.1±3.6)岁,填充含rhbFGF和rhBMP-2的骨水泥。比较3组Oswestry 功能障碍指数(Oswestry dysfunction index,ODI)、骨密度、椎体前缘丢失高度、伤椎前缘压缩率、疼痛视觉模拟评分(visual simulation score,VAS)及再骨折发生率。结果:所有患者获得12个月随访。3组术后ODI、VAS呈下降(P<0.001),骨密度增高(P<0.001),椎体前缘丢失高度、伤椎前缘压缩率呈先下降后缓慢上升趋势(P<0.001),rhbFGF+rhBMP-2组术后第1、6、12个月ODI、VAS均低于rhBMP-2组和磷酸钙组(P<0.05),术后第6、12个月骨密度大于rhBMP-2组和磷酸钙组(P<0.05)。rhbFGF+rhBMP-2组术后第6、12个月椎体前缘丢失高度、伤椎前缘压缩率均低于rhBMP-2组和磷酸钙组(P<0.05)。3组再骨折发生率比较差异无统计学意义(P>0.05)。结论:含rhbFGF和rhBMP-2骨水泥可更有效地增加OVCF患者骨密度,获得术后满意的临床和放射学效果,显著改善临床症状。 相似文献

目的:推荐使用胫骨外侧钢板。方法:将107例Ⅰ°、Ⅱ°开放性胫骨骨折随机分组,分别采用胫骨外侧钢板和普通加压钢板治疗,随诊1年~2.5年,比较两组术后功能和感染发生率。结果:发现两组功能评价无显著差异,但胫骨外侧钢板组感染发生率及骨髓炎发生率均明显低于普通加压钢板组(P<0.05)。结论:胫骨外侧钢板能明显降低开放性胫骨骨折的感染率和骨髓炎发生率,值得推荐。 相似文献

The aim of the present study was to investigate the influence of the size and the porosity of excipient microcrystalline cellulose (MCC) particles on the densification and the deformation during compaction and the consequent effect on the drug release from reservoir pellets. Drug pellets consisting of salicylic acid and microcrystalline cellulose were prepared by extrusion-spheronisation and spray-coated with ethyl cellulose (ethanol solution). Excipient pellets of different size and porosity were prepared by extrusion-spheronisation or direct spheronisation. Five binary mixtures of reservoir pellets and excipient particles were prepared in the proportion 1:7 and lubricated. After compaction the reservoir pellets were retrieved and analysed to determine the intragranular porosity, surface area, shape and drug release. The reservoir pellets were shown to undergo extensive deformation and densification during compaction, resulting in a preserved or even prolonged drug release time. The mode of deformation of the reservoir pellets seems to be critical for the compression-induced change in drug release. Formation of large indents has a negative effect on the release time, while the use of small particles or small deformable agglomerates has a protective effect. We also hypothesize that the coating structure changes during compaction and the final structure of the coating is the net effect of two parallel processes, one reducing and one prolonging the drug transport time across the coating. 相似文献

Compressive vertebral haemangiomas (VHs) are rare. Correct preoperative diagnosis is useful both for operative planning (since
compressive VHs are extremely vascular lesions) and to allow preoperative embolisation. Numerous radiological signs for VHs
have been described, but compressive VHs frequently have atypical features. In particular, magnetic resonance features are
not well established. We present imaging features in three cases of compressive VH and review the imaging findings in an additional
106 previously published cases. Findings were typical in 52 of 80 plain film (65 %), 33 of 41 computed tomography (80 %) and
13 of 25 magnetic resonance examinations (52 %). The prevalence of previously described imaging features is reported. Awareness
of the range of magnetic resonance features is important since this is frequently the initial investigation in patients presenting
with symptoms of neural compression. Since computed tomography is typical in 80 % of cases, this is a useful confirmatory
test if magnetic resonance features are suspicious but not diagnostic of compressive VH.

Acute oculomotor nerve palsy requires urgent exclusion of aneurysmal compression. We report a 62 year old man with a transient right third nerve palsy with pupillary involvement, who was found to have neurovascular compression of the cisternal oculomotor nerve as it curved over a duplicated superior cerebellar artery on high resolution MR imaging. 目的对单棉签平行血管按压法和双棉签并排垂直血管按压法的效果进行比较,选择更好的按压方式尽可能地减少重症肝炎患者在静脉采血后可能出现的皮肤出血、血肿、皮下瘀斑等不良情况,减轻病患的痛苦。方法将重症肝炎患者与普通患者分为观察组与对照组,在组内又分别采用单棉签平行血管按压法和双棉签并排垂直血管按压法进行止血,并记录各组的反应情况。结果观察组内采用双棉签并排垂直血管按压法,皮肤出血、血肿、皮下瘀斑的概率明显低于采用单棉签平行血管按压法(P<0.01),差异具有统计意义;而对照组内采用两种按压方式出现不良反应的概率差异无统计学意义。结论对普通患者而言,两种按压方式的差别不大;但对于重症肝炎患者来说,双棉签并排垂直血管按压法能够有效地降低皮肤出血、血肿和皮下淤斑的发生率,进而达到更好的治疗和护理效果。 相似文献

Antibiotic-loaded acrylic bone cements (ALABC) spacers are routinely used in the treatment of prosthetic joint infections. The objectives of our study were to evaluate different ALABC for elution kinetics, thermal stability, and mechanical properties. A 10 or 20% mixture (w/w) beads of medium viscosity bone cement (DePuy, Inc) and vancomycin (VAN), gentamycin (GM), daptomycin (DAP), moxifloxacin (MOX), rifampicin (RIF), cefotaxime (CTX), cefepime (FEP), amoxicillin clavulanate (AmC), ampicillin (AMP), meropenem (MER), and ertapenem (ERT) were formed and placed into wells filled with phosphate-buffered saline. Antibiotic concentrations were determined using high-performance liquid chromatography. Antimicrobial activity was tested against Micrococcus luteus ATCC 9341 or Escherichia coli ATCC 25922. AmC, AMP, and FEP concentration rapidly decreased after day 2, being almost undetectable at day 4. Sustained and high elution rates were observed with VAN, GM, MOX, and RIF for the 30-day duration of the experiment. DAP, MER, ERT, and CTX elution rates constantly decreased from day 4. All antibiotics tested retained antimicrobial activity proving thermal stability. Mechanical properties of ALABC were maintained except when RIF was used. 相似文献
