目的本研究的目的在于评价自制同种异体天然细胞外基质(Nature Extracellular Matrix,NECM)在修复兔耳廓软骨缺损中的价值,从而在体外研究的基础上进一步证实我们采用的去污剂-酶四步法制作的NECM在组织工程学中的可行性.方法选用10只新西兰大白兔,于其双耳廓制造1.0×.0cm大小的软骨缺损后,分别作以下研究:①灰兔NECM修复兔耳廓缺损.②灰兔NECM复合软骨膜修复兔耳廓缺损;③不植任何材料的空白对照.术后4、6、12、24周取材并行组织学检查.结果NECM植入后4、6周有轻重不一的炎症反应,第12周炎症反应消失,NECM周边无包裹现象.第12周时NECM复合软骨膜组有软骨膜细胞长入NECM,此时的软骨膜细胞多处于增殖期,24周时软骨膜细胞继续增殖分化细胞逐渐长入NECM深部,且部分软骨膜细胞发育成熟.单纯NECM修复组,第6周出现纤维结缔组织长入NECM,12、24周纤维结缔组织逐渐长入NECM深部,并逐渐取代NECM.结论①NECM可诱导软骨膜细胞增殖分化生长,修复软骨缺损:②NECM具有较好的组织亲和性和相容性;③软骨膜细胞可以作为NECM再生软骨的种子细胞的来源;④软骨膜可以阻止纤维结缔组织侵入修复区NECM. 相似文献

目的 探索工程兵矽肺患者肺功能与其胸片X线表现的关系。方法 选择 112例工程兵矽肺患者就其肺功能与胸片X线表现的关系进行统计分析。结果 Ⅰ、Ⅱ期患者中通气功能限制性和阻塞性障碍较多 ,分别占 75 %和 5 0 % ,Ⅲ期以混合性障碍为主占 70 % ;肺功能损伤Ⅰ、Ⅱ期以轻度为主 ,分别占 71%和 5 0 % ,Ⅲ期以中、重度为主 ,占 90 % ;肺功能损伤程度及肺功能损伤分型与矽肺阴影的肺区分布的数量有关。结论 随着矽肺阴影分布范围的增加以及期别的增高 ,肺损伤程度加重 ,肺功能障碍分型也逐渐由限制性 /阻塞性向混合性通气功能障碍转变。 相似文献

ObjectiveEndoplasmic reticulum (ER) aminopeptidase 1 (ERAP1) variants contribute to the risk of ankylosing spondylitis in HLA-B27 positive individuals, implying a disease-related interaction between these gene products. The aim of this study was to determine whether reduced ERAP1 expression would alter the cell surface expression of HLA-B27 and the formation of aberrant disulfide-linked forms that have been implicated in the pathogenesis of spondyloarthritis.MethodsERAP1 expression was knocked down in monocytic U937 cells expressing HLA-B27 and endogenous HLA class I. The effect of ERAP1 knockdown on the accumulation HLA-B alleles (B18, B51, and B27) was assessed using immunoprecipitation, isoelectric focusing, and immunoblotting, as well as flow cytometry with antibodies specific for different forms of HLA-B27. Cell surface expression of aberrant disulfide-linked HLA-B27 dimers was assessed by immunoprecipitation and electrophoresis on non-reducing polyacrylamide gels.ResultsERAP1 knockdown increased the accumulation of HLA-B27 on the cell surface including disulfide-linked dimers, but had no effect on levels of HLA-B18 or -B51. Antibodies with unique specificity for HLA-B27 confirmed increased cell surface expression of complexes shown previously to contain long peptides. IFN-γ treatment resulted in striking increases in the expression of disulfide-linked HLA-B27 heavy chains, even in cells with normal ERAP1 expression.ConclusionsOur results suggest that normal levels of ERAP1 reduce the accumulation of aberrant and disulfide-linked forms of HLA-B27 in monocytes, and thus help to maintain the integrity of cell surface HLA-B27 complexes. 相似文献

AbstractObjectives. This 2013 update of the practice guidelines for the biological treatment of unipolar depressive disorders was developed by an international Task Force of the World Federation of Societies of Biological Psychiatry (WFSBP). The goal has been to systematically review all available evidence pertaining to the treatment of unipolar depressive disorders, and to produce a series of practice recommendations that are clinically and scientifically meaningful based on the available evidence. The guidelines are intended for use by all physicians seeing and treating patients with these conditions. Methods. The 2013 update was conducted by a systematic update literature search and appraisal. All recommendations were approved by the Guidelines Task Force. Results. This first part of the guidelines (Part 1) covers disease definition, classification, epidemiology, and course of unipolar depressive disorders, as well as the management of the acute and continuation phase treatment. It is primarily concerned with the biological treatment (including antidepressants, other psychopharmacological medications, electroconvulsive therapy, light therapy, adjunctive and novel therapeutic strategies) of adults. Conclusions. To date, there is a variety of evidence-based antidepressant treatment options available. Nevertheless there is still a substantial proportion of patients not achieving full remission. In addition, somatic and psychiatric comorbidities and other special circumstances need to be more thoroughly investigated. Background & aimsSarcopenia is a clinical syndrome that features muscle atrophy and weakness, and has been associated with cardiovascular events and poor clinical outcomes. Recently, the sarcopenia index (SI) was developed as a simple screening tool based upon the serum creatinine to cystatin C (CysC) ratio. We investigated the association between SI and the prevalence of major adverse cardiovascular events (MACE) in patients with obstructive CAD.Methods & ResultsBetween January 2010 and December 2018, patients with angina pectoris and obstructive CAD requiring coronary artery intervention were enrolled. Serum levels of CysC and other biomarkers were assessed. Patients were divided into two groups according to the SI ([Cr/CysC] x 100). Demographic characteristics and clinical outcomes of the two groups were evaluated. A total of 427 patients (79.6% men, mean age 69.55 ± 12.04 years) were enrolled. Patients with SI < 120 (n = 214, 28%) were older, more likely to be of the female gender, and to have more hypertension and congestive heart failure (all p < 0.05). The prevalence of major adverse cardiovascular events (MACE) composed of myocardial infarction, stroke, and all-cause mortality was higher in patients with lower SI (p = 0.026). After adjusting for potential confounding factors, multivariate Cox regression (hazard ratio 2.08, p = 0.045) and Kaplan–Meier analyses (log-rank p = 0.0371) revealed that lower SI was significantly associated with a higher prevalence of MACE.ConclusionsSerum creatinine to cystatin C ratio (SI) may be a useful surrogate marker to predict the future prevalence of MACE in patients with obstructive CAD. 相似文献

Depression is increasingly prevalent in Western countries. It has severe consequences and is associated with increased rates of disability, morbidity, and mortality. Despite numerous therapeutic options, a great number of depressed patients do not achieve full remission. In addition, despite good short-term outcomes, long-term therapeutic results remain disappointing and associated with a poor prognosis, raising significant concern in terms of public health. Impaired sleep – especially insomnia – may be at least partly responsible for this problem. Very close relationships between major depressive disorder (MDD) and sleep disorders have been observed. In particular, residual symptoms of sleep disturbance in a remitted patient may predict a relapse of the disease. However, most currently available antidepressants do not always take into consideration the sleep disturbances of depressed patients; some agents long used in clinical practice even appear to worsen them by their sleep-inhibiting properties. But some other new medications were shown to relieve early sleep disturbance in addition to alleviating other depression-related symptoms. This positive impact should promote compliance with medication and psychological treatments, and increase daytime performance and overall functioning. Complete remission of MDD appears therefore to depend on the relief of sleep disturbances, a core symptom of MDD that should be taken into consideration and treated early in depressed patients. 相似文献
