带蚓状肌的游离足趾再造拇、手指

自１９７２年以来，在游离足趾再造拇、手指手术中，应用足趾蚓状肌与缺损拇、手指蚓状肌残端或关节囊进行缝合的方法共完成手术３００例，由于再造拇、手指的蚓状肌得到修复，既改善了其指间关节屈曲畸形，又防止了掌指关节的过伸。术后随访１－２２年，再造拇、手指的外形及功能均满意。     

肺内小病灶CT表现及其病理基础的研究：附76例分析

本文对７６例肺内小病灶（直径≤３ｃｍ）病例进行了ＣＴ－病理对照研究。结果表明，短毛刺征，深分叶征、棘状突起征、空泡征、血管集束征，胸膜凹陷征等对于诊断周围型小肺癌有重要临床价值；而尖角征、局限性胸膜肥厚粘连对诊断良性病灶有意义。本文重点讨论了血管集束征的定义，ＣＴ表现及病理基础；并就有争议的胸膜凹陷征提出一些粗浅的看法。旨在提高肺内小病灶良恶性诊断的准确率     

Model parameters of the smooth pursuit eye movement system with electrooculogram

The visual pursuit test is a method that collects and analyzes the characteristics of pursuit eye movements and examines the function of the eye movement system. This paper analyzes the model parameters of the smooth pursuit eye movement system in order to explore a method for improving the analysis. The input-output relationship of the smooth pursuit system can be expressed by a quasilinear model. We compute the model parameters (gain, phase, spectral purity, cross covariance) by digital signal processing. Eye movement is recorded by electrooculogram. Both eyes are tested individually. The visual target moves at frequencies of 0.2, 0.4, 0.8, 1.2, and 1.6 Hz. Ranges are gain, 1.01 to 0.70; phase, -0.1 ° to -66 °; spectral purity, 0.97 to 0.70; and cross covariance, 0.99 to 0.26. We tested 40 normal subjects as well as patients with ataxia (8), vertigo (18), and ophthalmoplegia (9). The oculomotor system of normal subjects functions as a linear system in the performance of this test at 0.2 to 0.8 Hz. The spectral purity dropped to about 0.70 at 1.6 Hz. The variability of all measures increases greatly at 1.6 Hz, which indicates that this target motion exceeds the tracking ability of many normal subjects and that the oculomotor system of normal subjects functions as a nonlinear system in this condition. Statistical tests show no significant differences between sex, age, and the two eyes. The model parameters tentatively proved effective in clinical application.     

免疫磁性捕获ELISA法检测甘草中甘草蛋白的研究

目的 制备甘草蛋白免疫磁性微球，并建立快速、精确的免疫磁性捕获ELISA法检测甘草蛋白。方法 采用种子聚合法合成聚苯乙烯磁性微球，并以兔抗甘草蛋白IgG抗体致敏，制备特异性捕获甘草特征蛋白的免疫磁性微球。以生物素标记抗体为示踪抗体，结合辣根过氧化物酶标亲和素建立ELISA检测系统，用于甘草药材和含甘草中成药中甘草蛋白的分析。结果 利用该方法对甘草药材和中成药中甘草蛋白抗原检测，检测灵敏度达到10ng／mL。结论 免疫磁性捕获ELISA检测技术方便、快速、准确，为生药的品种鉴定及中成药的质量控制提供一种新方法。     

经皮椎体成形术治疗脊柱疾病

目的评价经皮椎体成形术治疗脊柱疾病的临床应用价值.方法 56例多发骨髓瘤、溶骨性脊柱转移瘤、骨质疏松性椎体压缩性骨折患者应用经皮椎体成形术后,分24 h、3个月两阶段评估患者疼痛、术后X线片检查结果、椎体高度等指标.结果术后止痛效果良好,尤以骨质疏松性椎体压缩性骨折患者的止痛效果最好.无严重并发症. 结论经皮椎体成形术对骨质疏松性椎体压缩性骨折等脊柱疾病的止痛、稳固椎体等效果明显,可以谨慎开展.     

脾切除贲门周围血管离断术后肝脏血流动力学和储备功能的变化

Objective To observe changes of hepatic hemodynamics and hepatic functional reserve after splenectomy and periesophagogastric devascularization. Methods From July 2006 to August 2007, thirty patients with portal hypertension caused by hepatitis B cirrhosis underwent splenectomy and periesophagogastric devascuiarization. The PVPG (portal venous pressure gradient) was measured by inductor continually during operation. The HAF (hepatic artery flow), PVF (portal venours flow) and hepatic arterial RI(resistant index) were measured with Doppler sonography. The EHBF(effective hepatic blood flow) and ICGR15 (indocyaninegreen retention rate at 15 minutes) were obtained respectively by indocyaninegreen clearance test before and after the operation. Results PVPG after laparotomy (19±4) mm Hg, ligating the splenic artery(14±4) mm Hg, splenectomy(14±3)mm Hg and periesophagogastric devascularization (12±4) mm Hg showed a tendency to decrease progressively. The PVF decreased [from (42±14) ml/s to (16±8) ml/s] and HAF increased in compensation after operation. The EHBF increased [from (0.48±0.10) L/min to (0.56±0.10) L/min], and the ICGR15 decreased (from 23%±8% to 18%±4%) postoperatively. Conclusion After splenectomy and periesophagogastric devascularization, the hepatic functional reserve improves at least in a short term notwithstanding the decrease of PVPG and PVF.

目的探讨病案首页信息标准化建设.方法根据现行病案首页信息统计分析,对有关标准不一致处提出建议.结果对病案首页信息逐项提出要求及有关标准.结论病案首页信息的各项目必须统一要求、统一标准,以便医院间病案信息交流与资源共享.     

外伤性肝破裂的救治

目的 分析创伤性肝破裂外科多种干预治疗的效果。方法 总结分析1982-2002年我院收治创伤性肝破裂76例的救治方法和病死率。其中Ⅲ级以上的严重肝破裂46例(60．53％)。手术治疗56例，手术方式包括单纯修补、清创性肝切除、规则性肝切除、肝周填塞止血；非手术治疗20例。结果 手术组：治愈44例，术后并发症17例均经保守治疗治愈，死亡12例，其中术中死亡2例；非手术组：治愈12例，好转6例，2例失访。全组治愈64例，死亡12例，病死率15．8％。结论 Ⅰ-Ⅱ级单纯性外伤性肝破裂可保守治疗；手术是治疗创伤性肝破裂的主要措施，正确的手术方式，积极处理合并伤，重视综合治疗可提高救治成功率。     

磨损微粒诱导的局部生物学反应

人工髋关节置换失败的主要原因是假体的无菌性松动。研究表明 ,这和假体长期磨损产生的微粒作用于周围的巨噬细胞而诱发的生物学反应有关。磨损微粒激活假体周围组织细胞释放前炎症介质 ,诱导破骨细胞的活化和分化 ,引发假体周围的骨溶解。充分地认识这个过程 ,对减少人工关节无菌性松动 ,延长假体使用寿命具有重要意义。     

氟对体外培养人牙乳头间充质细胞合成I型胶原的影响

目的:明确氟对人牙乳头细胞合成 I型胶原的影响。 方法:对人牙乳头细胞进行体外培养 ,分别给予0 .0、0 .2× 10 - 6、1.0× 10 - 6、5 .0× 10 - 6、2 5 .0× 10 - 6 g/ L 氟处理 ,采用免疫细胞化学方法染色 ,图象分析氟对细胞合成 I型胶原的影响。 结果:I型胶原在对照组和 4种氟浓度处理组细胞表达均为阳性。图象分析结果表明 ,4种氟浓度对体外培养的人牙乳头细胞合成 I型胶原量与对照组相比差异无统计学意义 (P>0 .0 5 )。结论:过量氟对牙本质基质 型胶原的影响可能不是通过影响成牙本质细胞内合成实现的
