观察循证护理在改善骨折患者疼痛、心理状态和生活质量中的应用效果。选取2014年2月至2015年6月我院骨折患者120例,按随机数字表法分为观察组及对照组,各60例。对照组进行常规护理,观察组在对照组基础上给予循证护理,记录并分析护理后2组患者的疼痛、心理状态和生活质量情况。观察组患者生理功能、社会功能、躯体疼痛、精神健康、总体健康评分显著优于对照组(P<0.05)。观察组术后第1天、术后第7天、出院前疼痛评分值均显著优于对照组(P<0.05)。循证护理在改善骨折患者疼痛、心理状态和生活质量中的效果确切,值得临床进一步研究和应用。  相似文献

推行家庭医生签约服务是实施分级诊疗、保障人民健康的重要途径,也是实现“健康中国2030”的重要基石。但是,家庭医生“签而不约”等问题制约了这一制度的发展。国际上许多国家已在签约模式中不同程度地引入竞争机制,以提高家庭医生服务质量。然而,国内目前尚缺乏基于家庭医生签约背景 引入竞争机制的理论与实证研究。本文从家庭医生签约服务引入竞争机制这一发展方向入手,梳理了各国相关理论和实证研究进展,以期为我国家庭医生签约制度研究和实践提供依据。  相似文献

Interactions between biological macromolecules and micrometer- and sub-micrometer-scale surface structures are directly influenced by the surface wettability, chemical reactivity and surface charge. Understanding these interactions is crucial for developing integrated microsystems for biological and biomedical processing and analysis. We report development of selective surface modification techniques based on microcontact printing and polyelectrolyte adsorption. These techniques were applied to lithographically patterned silicon microfluidic channels and flat silicon substrates to create surface microstructures with contrasting wetting properties and surface charges. These controls enabled us to devise various techniques for controlled loading and processing of biomaterials in the channels. Solutions containing long chain biological macromolecules DNA and microtubules were directly loaded into the microchannels by using a micromanipulator/microinjector system. Structural arrangements of these linear macromolecules, which were probed by using fluorescence and laser scanning confocal microscopy, were found to be quite different from bulk solutions. As expected, the filamentous molecules were observed to align linearly along the channels, with the degree of alignment dependent on channel width as well as the length of the molecule. This molecular alignment, which is induced by both the surface confinement effect and capillary flow during sample loading, may be used to enhance processing of biological materials in silicon biomedical microdevices. It also opens up the possibility of carrying out direct combinatorial structural characterization of proteins in the microchannels utilizing X-ray diffraction, which so far has not been possible.  相似文献

目的:评价白内障手术方式及Acrysof人工晶状体植入手术的疗效。方法:对3组61例(64眼)老年性白内障分别采用3.2-6.5mm巩膜隧道和孤长12mm巩膜阶梯切口,分别施行白内障超声乳化摘出联合折叠式人工晶状体植入术,超声乳化摘出联合一体型人工晶状体植入术和囊外摘出联合一体型人工晶状体植入术。结果:术后5天3组平均裸眼视力分别为0.83,0.75和0.51,达到1.0以上视力比例分别为53.33%,26.09%和7.69%,前者和后两者比较,差异均有显著性(q检验,P<0.01)。结论:采用小切口折叠式人工晶状体植入术具有术后视力早期恢复的优点。  相似文献

A rapid and sensitive method based on liquid chromatography/tandem mass spectrometry (LC/MS/MS) was developed for the simultaneous determination of lithospermic acid B and its three main O-methylated metabolites in rat serum with silibinin as the internal standard. The calibration curves for LSB, and the three metabolites were linear over the ranges of 16–4096, and 8–2048 ng/ml, respectively, with coefficients of correlation >0.998. For LSB, the intra-assay coefficient of variance (CV) was less than 9.3% and the inter-assay CV was less than 8.9%. The inter-assay mean accuracy was between 92.8% and 104.7%. For the three metabolites, the intra-assay CV was less than 8.7% and the inter-assay CV was less than 9.9%. The inter-assay mean accuracy was between 92.5% and 107.9%. This quantitation method was successfully applied to a pharmacokinetic study of LSB in rats. Also, a total recovery of 5.2%was found in bile after oral administration.  相似文献

BACKGROUND: In the process of vascularization, vascular endothelial growth factor (VEGF), angiopoietin-2 and Tie2 are involved in the migration, differentiation and proliferation of vascular endothelial cells, and stimulate the rapid angiogenesis; Tiel and angiopoietin-1 play important roles in facilitating the formation of vascular lumen and maintaining the integrity of vascular wall. Thus the distributions and expressions may be associated with the occurrence of cerebral arteriovenous malformation. OBJECTIVE: To observe the biological effects of angiogenic factors in the occurrence and development of cerebral arteriovenous malformation. DESIGN: An observational comparative experiment. SETTINGS: Department of Neurosurgery, General Hospital of Shenyang Military Area Command of Chinese PLA; Department of Neurosurgery, General Hospital of Tianjin Medical University. PARTICIPANTS: Fresh samples of complete cerebral arteriovenous malformations resected in 47 patients were collected from the Department of Neurosurgery, General Hospital of Tianjin Medical University from August 1999 to May 2001, including 22 males and 25 females, the mean age was 34.5 years. Informed consents were obtained from all the patients or their relatives. The initial symptom was hemorrhage in 28 cases. All the patients were classified according to the clinical imaging data and Spetzler-Martin grading standard, including 11 cases of grade Ⅰ, 17 cases of grade Ⅱ, 11 cases of grade Ⅲ, and 8 cases of grade Ⅳ - Ⅴ. Normal brain tissues resected by decompression due to trauma were taken from 8 patients as controls, including 5 males and 3 females, aging 12 - 65 years. METHODS: ① The expressions of VEGF, Tie receptors, angiopoietin-1, angiopoietin-2, proto-oncogene c-myc and proliferating cell nuclear antigen(PCNA) in the samples of cerebral arteriovenous malformation were detected with immunohistochemical method. Under light microscope, the positively stained rat-anti-human factor Ⅷ-related antigens (specific marker of vascular endothelial cells) were counted, then the immuno-positive cells of the other antibodies in the visual field of neighboring section which was in "mirror" relation were counted, and the percentage of the latter to the former was considered as the labeling index of positive cells. The immunostaining intensity was classified negative ( - ): no positive cells; positive (+): number of positive cells 〈 20%; moderately positive (++): number of positive cells 20% - 50%; strongly positive (+++): number of positive cells 〉 50%. ② The differences of the enumeration data were compared with chi-squam test, and the correlation were analyzed with the linear correlation analysis. MAIN OUTCOME MEASURES: Expressions and distributions of VEGF, Tie 1 and Tie2 receptors, angiopoietin-1, angiopoietin-2, PCNA and c-myc in the samples of cerebral arteriovenons malformation and normal brain tissue. RESULTS: ① Expressions of angiogenic factors in the control group and cerebral arteriovenons malformation groups of each grade: The positive rates of VEGF, Tie2, angiopoietin-2, c-myc and PCNA expressions in the control group were significantly different from those in the cerebral arteriovenous malformation groups of each grade ( x^2=21.09 - 34.23, P 〈 0.05), whereas the positive rates of Tiel and angiopoietin-1 expressions were close ( x^2=3.43 - 3.869, P 〉 0.05). ② Expressions of angiogenic factors in hemorrhage group and non-hemorrhage group: The expressions of VEGF, angiopoietin-2 and PCNA in the hemorrhage group were significantly lower than those in the non-hemorrhage group ( x^2= 16.22 - 26.56, P 〈 0.05). There ware no obvious differences in the expressions of Tiel and angiopoietin-1 expressions between the hemorrhage group and non-hemorrhage group ( x^2=3.22 - 3.78, P 〉 0.05).The VEGF was positively correlated with the expressions of c-myc and PCNA (r = 0.728, 0.916, P 〈 0.05). CONCLUSION: ①The expressions of angiogenic factors and related receptors may be involved in the process of cerebral arteriovenous malformation, and had important correlation the its clinical grading. ② Angiogenic factors may induce the expression of endothelial cell c-myc in cerebral arteriovenous malformation, and then interfere the cell proliferation and apoptosis.  相似文献

Aminoacyl-tRNA synthetases (AARS) are a family of enzymes that exhibit primary and various secondary functions in different species. In Brugia malayi, the gene for asparaginyl-tRNA synthetase (AsnRS), a class II AARS, previously has been identified as a multicopy gene encoding an immunodominant antigen in the serum of humans with lymphatic filariasis. However, the relative level of expression and alternative functions of AARS in nematode parasites is not well understood. We searched the Filarial Genome Project database to identify the number and amino acid specificity of B. malayi AARS cDNAs to gain insight into the role of different AARS in filaria. These data showed that cytoplasmic AsnRS was present in five gene clusters, and is the most frequently represented member of the aminoacyl-tRNA synthetase family in adult B. malayi. The relative level of AsnRS transcribed in adult female B. malayi was compared to the levels of a low abundance and medium abundance AARS by quantitative real-time RT-PCR. By this method, AsnRS cDNA was 11 times greater than arginyl-tRNA synthetase and methionyl-tRNA synthetase cDNA. In situ hybridization using a B. malayi AsnRS-specific oligonucleotide probe identified abundant cytoplasmic mRNA, particularly in the hypodermis of adult B. malayi. In the absence of tRNA, AsnRS synthesizes diadenosine triphosphate, a potent regulator of cell growth in other eukaryotes. These data support the hypothesis that all AARS are not equally expressed in B. malayi and that these enzymes may demonstrate important alternative functions in filaria.  相似文献

Helicobacter pylori strains possessing the cag pathogenicity island (PaI) are associated with the development of gastroduodenal diseases, including gastric cancer. cag PaI products induce the secretion of interleukin-8 (IL-8) from epithelial cells and facilitate the translocation of CagA into the cell cytosol. In East Asia, where the incidence of gastric cancer is high, most strains possess the cag PaI. To date, however, no cag PaI phenotypic data have been provided for strains isolated in mainland China. Here we used 31 Chinese strains to determine the genotypic and phenotypic status of the cag PaI. All strains possessed cagA and cagE, and we observed a variation in the length of cagA variable regions. Nucleotide sequencing of the cagA variable region revealed that CagA was of two types, a short "Western" form with two tyrosine phosphorylation sites and a longer "East Asian" form with three tyrosine phosphorylation sites. Coculture of strains with AGS epithelial cells showed that strains could induce IL-8 secretion from the cells and that CagA with three phosphorylation sites became more phosphorylated than that with two and could induce significantly (P < 0.001) more cells to elongate. We hypothesize that the preponderance of the more active East Asian form of cagA may underlie the high rate of gastric cancer in China.  相似文献

目的 研究低分子量肝素 -速避凝对白内障术后眼内炎症反应的预防作用。方法  110例老年性白内障随机分为 2组 ,并盲法给药。在进行白内障囊外摘除及后房人工晶体植入手术中 ,对照组灌注液为乳酸钠林格氏液 ,实验组灌注液为乳酸钠林格液中加入速避凝浓度为 6U/ml。术后 1、3及 7d观察眼内纤维蛋白渗出、房水混浊及眼内出血情况。结果 实验组术后第 1、3、7d眼内纤维蛋白渗出及房水混浊轻于对照组 (P <0 .0 5 ) ,两组术中及术后出血差异无显著性 (P >0 .1)。结论 速避凝能有效抑制白内障术后眼内炎症反应 ,并且不增加眼内出血危险性。  相似文献

目的 分析踝关节骨折的手术方法和内固定技术特点。方法 对102例踝关节骨折的手术治疗的临床资料进行回顾性分析,强调外踝解剖复位及下胫腓损伤灵活处理,对后踝、内踝合理内固定并注意距骨体在踝穴中的位置。结果 随访8个月至4年以上,优54例,良38例,可9例,差1例,优良率90.2%。结论 骨折手术治疗应遵循踝关节的生理特点,恢复踝关节的稳定,灵活合理有效施行内固定。  相似文献
