156. Stabilere Fixateur externe Osteosynthesen durch Einsatz eines neu entwickelten Schraubennagels

Zusammenfassung Frühere Untersuchungen hatten gezeigt, daß die bisher verwendeten 4,5 mm Steinmann-Nägel mit zentralem Gewinde die schwächsten Teile der Fixateur externe Systeme darstellen. Daher entwickelten wir einen abgesetzten Schraubennagel (4,5 bzw. 7 mm) mit AO-Corticalisgewinde. In experimentellen Untersuchungen waren Montagen mit neuen Schraubennägeln bis zu 40% stabiler als vergleichbare Montagen mit normalen Steinmann-Nägeln. Erste klinische Anwendungen waren erfolgreich. In Analogie zu unserem Nagel wurde eine modifizierte Schanzsche Schraube (5 bzw. 8 mm) entwickelt.     

Respiratory failure due to pulmonary leukostasis following chemotherapy of acute nonlymphocytic leukemia

Four patients with acute nonlymphocytic leukemia and leukocyte counts of more than 200,000/mm3 developed respiratory distress due to pulmonary leukostasis within 10-48 hours after initiation of chemotherapy. Clinically, the patients manifested fever, dyspnea, tachypnea, diffuse pulmonary rales, pleural effusions, and severe hypoxemia. Chest roentgenograms displayed diffuse pulmonary infiltrates, vascular engorgement, cardiomegaly, and pleural effusions. Three patients died from progressive respiratory failure despite ventilatory support. Pulmonary histology revealed thrombi composed of leukemic blast cells which obstructed and distended the lumens of pulmonary arterioles, capillaries, and venules. Electron microscopy studies of lung tissue showed pulmonary alveolar endothelium and basement membrane damage and interstitial edema. The pathophysiologic basis of pulmonary leukostasis and potential treatment modalities are discussed.     

Fixateur externe — Verhinderung der Seitverschiebung

The usually for the assemblage of the external pin fixation used Steinman pins loose themselves already after a short time — caused by local osteolyses — and displace in the borehole because of the lateral acting pressure. This phenomenon even cannot be prevented by using Steinman pins with central thread. To avoid this lateral deflection we are going to introduce as a model a screw-nail, which thread is constructed in such a way that a sufficient retention can be obtained in the bone.     

Über die Beziehungen zwischen dem Druck und der Stromstärke in der Arteria pulmonalis sowie der Leistung des rechten Ventrikels beim „unbeeinflußten Kreislauf“ der Katze und im oligämischen Schock

Zusammenfassung Die Druck-Stromstärkebeziehung in der Arteria pulmonalis und die Leistung des rechten Herzens wurden beim unbeeinflußten Kreislauf und im oligämischen Schock gemessen, sowie der Strömungswiderstand und der relative Gefäßquerschnitt berechnet. Die an Katzen durchgeführten Untersuchungen ergaben folgende Ergebnisse:1. Die Druckstromstärkekurve entspricht bei beiden untersuchten Kreislaufzuständen einer Geraden. Die Kurve des oligämischen Schocks ist gegenüber der des unbeeinflußten Kreislaufs im Koordinatensystem nach links zu kleineren Stromstärken und niedrigeren Druckwerten verschoben. Die Beziehung zwischen der wirksamen Druckdifferenz (mittlerer arterieller Druck — linker Vorhofdruck) folgt ebenfalls einer Geraden.2. Im oligämischen Schock ist die Leistung des rechten Herzens herabgesetzt (Einschränkung der Druckentwicklung und des Minutenvolumens bei erhöhtem rechten Vorhofdruck).3. Der Strömungswiderstand ist im oligämischen Schock kleiner als beim unbeeinflußten Kreislaufzustand. Er nimmt mit steigendem Druck ab. Der relative Gefäßquerschnitt verhält sich gegensinnig.4. Die Pulsfrequenz nimmt mit steigendem Blutvolumen in der Mehrzahl der Versuche ab. Sie ist im oligämischen Schock deutlich niedriger als beim unbeeinflußten Kreislauf.Mit 3 Textabbildungen     

Evaluation and biomechanics of the first ray in the patient with limited motion

Adequate first ray function is essential to healthy human gait. Controversies still exist about aspects of human structure and function and many newer answers and theories have been proposed by a new generation of experts. Examples include the sagittal plane facilitation, tissue stress, and preferred movement pathway theories. This article also presents a summary of how to provide a thorough, detailed, and accurate first ray examination on the individual with limited motion. This article explores functional first ray mechanics in both a theoretic and biomechanical perspective, as well as a practical, hands-on examination perspective.     

Detection of the third heart sound using a tailored wavelet approach: Method verification

Heart sounds can be considered as mechanical fingerprints of myocardial function. The third heart sound normally occurs in children but disappears with maturation. The sound can also appear in patients with heart failure. The sound is characterised by its low-amplitude and low-frequency content, which makes it difficult to identify by the traditional use of the stethoscope. A wavelet-based method has recently been developed for detection of the third heart sound. This study investigated if the third heart sound could be identified in patients with heart failure using this detection method. The method was also compared with auscultation using conventional phonocardiography and with characterisation of the patients with echocardiography. In the first study, 87% of the third heart sounds were detected using the wavelet method, 12% were missed, and 6% were false positive. In study 2, the waveletdetection method identified 87% of the patients using the third heart sound, and regular phonocardiography identified two (25%) of the subjects.
