We examined the perioperative balance between oxygen delivery and oxygen consumption under hemodilution in 24 patients who underwent head and neck surgery of long duration and with massive blood loss. Intraoperative mixed venous oxygen saturation (SVO2), which decreased according to a decrease in hematocrit (Hct), was almost always kept above 60% when Hct was more than 20%. The longer the duration of surgery, the lower was SVO2 at the end of surgery. Postoperative SVO2 correlated positively with SVO2 at the end of surgery, and negatively with the duration of surgery and blood loss, but did not correlate with Hct. In 9 cases, SVO2 at the recovery from anesthesia decreased to below 60%, because oxygen consumption increased remarkably with shivering. These results suggest that in highly invasive head and neck surgery, the intraoperative minimum acceptable Hct level is 20%. On the other hand, there is no general postoperative minimum acceptable Hct level, because postoperative oxygen demand and supply balance depends on the degree of surgical invasion and increase of VO2. SVO2 at the end of surgery is useful in predicting postoperative oxygen demand and supply balance.  相似文献

The effects of three different types of surfactant systems (ionic, polymeric, and electrosteric stabilizers) on the water sensitivity of poly(butyl acrylate‐co‐methyl methacrylate) latex films was examined. The water sensitivity was found to be strongly dependent on the surfactant system used in their preparation. A number of factors, such as the surfactant mobility and crystallinity and surfactant/polymer polarity appeared to affect the water uptake of the films. Highly mobile and crystallizable surfactants yielded high water sensitivity for films containing ionic surfactants, whereas the surfactant polarity had a greater effect on latices stabilized by polymeric surfactants, with the more hydrophilic systems providing greater water uptake. © 2004 Wiley Periodicals, Inc. A high-performance liquid chromatographic method for the determination of free reduced cysteine and N-acetylcysteine in human plasma at the basal state and after oral administration of N-acetylcysteine is described. The method is based on acid-catalysed conversion of plasma thiols to the corresponding S-nitroso derivatives by excess of nitrite and their subsequent cation-pairing RP-HPLC with detection at 333 nm. Recovery rates of cysteine and N-acetylcysteine added to human plasma were 94.6 and 99.6%, respectively. Inter- and intra-day precision were below 6%. In healthy humans (n = 5), free reduced cysteine was determined to be (mean+/-S.E.) 10.0+/-0.96 microM. No N-acetylcysteine was detected in plasma of these subjects above the limit of detection (e.g. 170 nM). The method was successfully applied to a pharmacokinetic study on orally administered N-acetylcysteine to healthy volunteers.  相似文献

BACKGROUND: The purpose of this study was to establish the norm for parameters of auditory brainstem response (ABR) in the guinea pig and to investigate if acute brainstem compression results in significant changes to these parameters. METHODS: Thirty-six guinea pigs with positive Preyer's reflex were anesthetized. A craniectomy was performed to remove the right occipital bone and the dura mater was opened to expose the brain, cerebellum and cerebellopontine angle (CPA). A small inflatable balloon was placed into the CPA precisely and slowly. ABR was recorded before incision of the skin as a baseline value, after placement and after inflation of the balloon with water at 0.1-ml intervals. RESULTS: Five stable peaks were recorded in 27 experimental animals. When the balloon was inflated with 0.1 ml water, the absolute latency (AL) of peaks IV and V and the interpeak latency (IPL) of peaks III and IV, and IV and V were prolonged. The amplitude ratios (AR) of peaks II, III, IV and V to peak I decreased. Inflation of the balloon with 0.2 ml of water caused further elongation of ALs of peaks IV and V and decreases in each AR. When the balloon volume increased to 0.3 ml, peak V became unrecognizable and peaks III and IV showed significant elongation of AL; peaks I and II did not show significant change in ALs. Further increase of the balloon volume to 0.4 ml resulted in disappearance of peaks III, IV and V; AL of peak II was also elongated. However, the amplitude and AL of peak I remained unchanged. Similar changes were observed in IPLs. CONCLUSIONS: This study establishes the norm of parameters of ABR in guinea pigs and demonstrates that acute brainstem compression causes elongation of ALs and IPLs of peaks II, III, IV and V. This suggests that peaks II, III, IV and V come from the brainstem and that peak I is not generated from the brainstem in the guinea pig.  相似文献

Clathrin-mediated endocytosis involves cycles of assembly and disassembly of clathrin coat components and their accessory proteins. Dephosphorylation of rat brain extract was shown to promote the assembly of dynamin 1, synaptojanin 1, and amphiphysin into complexes that also included clathrin and AP-2. Phosphorylation of dynamin 1 and synaptojanin 1 inhibited their binding to amphiphysin, whereas phosphorylation of amphiphysin inhibited its binding to AP-2 and clathrin. Thus, phosphorylation regulates the association and dissociation cycle of the clathrin-based endocytic machinery, and calcium-dependent dephosphorylation of endocytic proteins could prepare nerve terminals for a burst of endocytosis.  相似文献

The thermal and flammability properties of polypropylene/multi-walled carbon nanotube, (PP/MWNT) nanocomposites were measured with the MWNT content varied from 0.5 to 4% by mass. Dispersion of MWNTs in these nanocomposites was characterized by SEM and optical microscopy. Flammability properties were measured with a cone calorimeter in air and a gasification device in a nitrogen atmosphere. A significant reduction in the peak heat release rate was observed; the greatest reduction was obtained with a MWNT content of 1% by mass. Since the addition of carbon black powder to PP did not reduce the heat release rate as much as with the PP/MWNT nanocomposites, the size and shape of carbon particles appear to be important for effectively reducing the flammability of PP. The radiative ignition delay time of a nanocomposite having less than 2% by mass of MWNT was shorter than that of PP due to an increase in the radiation in-depth absorption coefficient by the addition of carbon nanotubes. The effects of residual iron particles and of defects in the MWNTs on the heat release rate of the nanocomposite were not significant. The flame retardant performance was achieved through the formation of a relatively uniform network-structured floccule layer covering the entire sample surface without any cracks or gaps. This layer re-emitted much of the incident radiation back into the gas phase from its hot surface and thus reduced the transmitted flux to the receding PP layers below it, slowing the PP pyrolysis rate. To gain insight into this phenomena, thermal conductivities of the nanocomposites were measured as a function of temperature while the thermal conductivity of the nanocomposite increases with an increase in MWNT content, the effect being particularly large above 160 °C, this increase is not as dramatic as the increase in electrical conductivity, however.  相似文献
