Elicitation of expert opinion is important for risk analysis when only limited data are available. Expert opinion is often elicited in the form of subjective confidence intervals; however, these are prone to substantial overconfidence. We investigated the influence of elicitation question format, in particular the number of steps in the elicitation procedure. In a 3‐point elicitation procedure, an expert is asked for a lower limit, upper limit, and best guess, the two limits creating an interval of some assigned confidence level (e.g., 80%). In our 4‐step interval elicitation procedure, experts were also asked for a realistic lower limit, upper limit, and best guess, but no confidence level was assigned; the fourth step was to rate their anticipated confidence in the interval produced. In our three studies, experts made interval predictions of rates of infectious diseases (Study 1, n = 21 and Study 2, n = 24: epidemiologists and public health experts), or marine invertebrate populations (Study 3, n = 34: ecologists and biologists). We combined the results from our studies using meta‐analysis, which found average overconfidence of 11.9%, 95% CI [3.5, 20.3] (a hit rate of 68.1% for 80% intervals)—a substantial decrease in overconfidence compared with previous studies. Studies 2 and 3 suggest that the 4‐step procedure is more likely to reduce overconfidence than the 3‐point procedure (Cohen's d = 0.61, [0.04, 1.18]).  相似文献

The mental models approach, a leading strategy to develop risk communications, involves a time‐ and labor‐intensive interview process and a lengthy questionnaire to elicit group‐level risk perceptions. We propose that a similarity ratings approach for structural knowledge elicitation can be adopted to assist the risk mental models approach. The LinkIT game, inspired by games with a purpose (GWAP) technology, is a ludic elicitation tool designed to elicit group understanding of the relations between risk factors in a more enjoyable and productive manner when compared to traditional approaches. That is, consistent with the idea of ludic elicitation, LinkIT was designed to make the elicitation process fun and enjoyable in the hopes of increasing participation and data quality in risk studies. Like the mental models approach, the group mental model obtained via the LinkIT game can hence be generated and represented in a form of influence diagrams. In order to examine the external validity of LinkIT, we conducted a study to compare its performance with respect to a more conventional questionnaire‐driven approach. Data analysis results conclude that the two group mental models elicited from the two approaches are similar to an extent. Yet, LinkIT was more productive and enjoyable than the questionnaire. However, participants commented that the current game has some usability concerns. This presentation summarizes the design and evaluation of the LinkIT game and suggests areas for future work.  相似文献

Physiological daily inhalation rates reported in our previous study for normal‐weight subjects 2.6–96 years old were compared to inhalation data determined in free‐living overweight/obese individuals (n = 661) aged 5–96 years. Inhalation rates were also calculated in normal‐weight (n = 408), overweight (n = 225), and obese classes 1, 2, and 3 adults (n = 134) aged 20–96 years. These inhalation values were based on published indirect calorimetry measurements (n = 1,069) and disappearance rates of oral doses of water isotopes (i.e., 2H2O and H218O) monitored by gas isotope ratio mass spectrometry usually in urine samples for an aggregate period of over 16,000 days. Ventilatory equivalents for overweight/obese subjects at rest and during their aggregate daytime activities (28.99 ± 6.03 L to 34.82 ± 8.22 L of air inhaled/L of oxygen consumed; mean ±  SD) were determined and used for calculations of inhalation rates. The interindividual variability factor calculated as the ratio of the highest 99th percentile to the lowest 1st percentile of daily inhalation rates is higher for absolute data expressed in m3/day (26.7) compared to those of data in m3/kg‐day (12.2) and m3/m2‐day (5.9). Higher absolute rates generally found in overweight/obese individuals compared to their normal‐weight counterparts suggest higher intakes of air pollutants (in μg/day) for the former compared to the latter during identical exposure concentrations and conditions. Highest absolute mean (24.57 m3/day) and 99th percentile (55.55 m3/day) values were found in obese class 2 adults. They inhale on average 8.21 m3 more air per day than normal‐weight adults.  相似文献

We reanalyzed the Libby vermiculite miners’ cohort assembled by Sullivan to estimate potency factors for lung cancer, mesothelioma, nonmalignant respiratory disease (NMRD), and all‐cause mortality associated with exposure to Libby fibers. Our principal statistical tool for analyses of lung cancer, NMRD, and total mortality in the cohort was the time‐dependent proportional hazards model. For mesothelioma, we used an extension of the Peto formula. For a cumulative exposure to Libby fiber of 100 f/mL‐yr, our estimates of relative risk (RR) are as follows: lung cancer, RR = 1.12, 95% confidence interval (CI) =[1.06, 1.17]; NMRD, RR = 1.14, 95% CI =[1.09, 1.18]; total mortality, RR = 1.06, 95% CI =[1.04, 1.08]. These estimates were virtually identical when analyses were restricted to the subcohort of workers who were employed for at least one year. For mesothelioma, our estimate of potency is KM = 0.5 × 10?8, 95% CI =[0.3 × 10?8, 0.8 × 10?8]. Finally, we estimated the mortality ratios standardized against the U.S. population for lung cancer, NMRD, and total mortality and obtained estimates that were in good agreement with those reported by Sullivan. The estimated potency factors form the basis for a quantitative risk assessment at Libby.  相似文献

Emergency vaccination is an effective control strategy for foot‐and‐mouth disease (FMD) epidemics in densely populated livestock areas, but results in a six‐month waiting period before exports can be resumed, incurring severe economic consequences for pig exporting countries. In the European Union, a one‐month waiting period has been discussed based on negative test results in a final screening. The objective of this study was to analyze the risk of exporting FMD‐infected pig carcasses from a vaccinated area: (1) directly after final screening and (2) after a six‐month waiting period. A risk model has been developed to estimate the probability that a processed carcass was derived from an FMD‐infected pig (Pcarc). Key variables were herd prevalence (PH), within‐herd prevalence (PA), and the probability of detection at slaughter (PSL). PH and PA were estimated using Bayesian inference under the assumption that, despite all negative test results, ≥1 infected pigs were present. Model calculations indicated that Pcarc was on average 2.0 × 10?5 directly after final screening, and 1.7 × 10?5 after a six‐month waiting period. Therefore, the additional waiting time did not substantially reduce Pcarc. The estimated values were worst‐case scenarios because only viraemic pigs pose a risk for disease transmission, while seropositive pigs do not. The risk of exporting FMD via pig carcasses from a vaccinated area can further be reduced by heat treatment of pork and/or by excluding high‐risk pork products from export.  相似文献

Lumber Liquidators (LL) Chinese‐manufactured laminate flooring (CLF) has been installed in >400,000 U.S. homes over the last decade. To characterize potential associated formaldehyde exposures and cancer risks, chamber emissions data were collected from 399 new LL CLF, and from LL CLF installed in 899 homes in which measured aggregate indoor formaldehyde concentrations exceeded 100 μg/m3 from a total of 17,867 homes screened. Data from both sources were combined to characterize LL CLF flooring‐associated formaldehyde emissions from new boards and installed boards. New flooring had an average (±SD ) emission rate of 61.3 ± 52.1 μg/m2‐hour; >one‐year installed boards had ∼threefold lower emission rates. Estimated emission rates for the 899 homes and corresponding data from questionnaires were used as inputs to a single‐compartment, steady‐state mass‐balance model to estimate corresponding residence‐specific TWA formaldehyde concentrations and potential resident exposures. Only ∼0.7% of those homes had estimated acute formaldehyde concentrations >100 μg/m3 immediately after LL CLF installation. The TWA daily formaldehyde inhalation exposure within the 899 homes was estimated to be 17 μg/day using California Proposition 65 default methods to extrapolate cancer risk (below the regulation “no significant risk level” of 40 μg/day). Using a U.S. Environmental Protection Agency linear cancer risk model, 50th and 95th percentile values of expected lifetime cancer risk for residents of these homes were estimated to be 0.33 and 1.2 per 100,000 exposed, respectively. Based on more recent data and verified nonlinear cancer risk assessment models, LL CLF formaldehyde emissions pose virtually no cancer risk to affected consumers.  相似文献

Average rates of total dermal uptake (Kup) from short‐term (e.g., bathing) contact with dilute aqueous organic chemicals (DAOCs) are typically estimated from steady‐state in vitro diffusion‐cell measures of chemical permeability (Kp) through skin into receptor solution. Widely used (“PCR‐vitro”) methods estimate Kup by applying diffusion theory to increase Kp predictions made by a physico‐chemical regression (PCR) model that was fit to a large set of Kp measures. Here, Kup predictions for 18 DAOCs made by three PCR‐vitro models (EPA, NIOSH, and MH) were compared to previous in vivo measures obtained by methods unlikely to underestimate Kup. A new PCR model fit to all 18 measures is accurate to within approximately threefold (r = 0.91, p < 10?5), but the PCR‐vitro predictions (r > 0.63) all tend to underestimate the Kup measures by mean factors (UF, and p value for testing UF = 1) of 10 (EPA, p < 10?6), 11 (NIOSH, p < 10?8), and 6.2 (MH, p = 0.018). For all three PCR‐vitro models, log(UF) correlates negatively with molecular weight (r2 = 0.31 to 0.84, p = 0.017 to < 10?6) but not with log(vapor pressure) as an additional predictor (p > 0.05), so vapor pressure appears not to explain the significant in vivo/PCR‐vitro discrepancy. Until this discrepancy is explained, careful in vivo measures of Kup should be obtained for more chemicals, the expanded in vivo database should be compared to in vitro‐based predictions, and in vivo data should be considered in assessing aqueous dermal exposure and its uncertainty.  相似文献

I introduce a model of undirected dyadic link formation which allows for assortative matching on observed agent characteristics (homophily) as well as unrestricted agent‐level heterogeneity in link surplus (degree heterogeneity). Like in fixed effects panel data analyses, the joint distribution of observed and unobserved agent‐level characteristics is left unrestricted. Two estimators for the (common) homophily parameter, β0, are developed and their properties studied under an asymptotic sequence involving a single network growing large. The first, tetrad logit (TL), estimator conditions on a sufficient statistic for the degree heterogeneity. The second, joint maximum likelihood (JML), estimator treats the degree heterogeneity {Ai0}i = 1N as additional (incidental) parameters to be estimated. The TL estimate is consistent under both sparse and dense graph sequences, whereas consistency of the JML estimate is shown only under dense graph sequences.  相似文献

This paper proposes a test for common conditionally heteroskedastic (CH) features in asset returns. Following Engle and Kozicki (1993), the common CH features property is expressed in terms of testable overidentifying moment restrictions. However, as we show, these moment conditions have a degenerate Jacobian matrix at the true parameter value and therefore the standard asymptotic results of Hansen (1982) do not apply. We show in this context that Hansen's (1982) J‐test statistic is asymptotically distributed as the minimum of the limit of a certain random process with a markedly nonstandard distribution. If two assets are considered, this asymptotic distribution is a fifty–fifty mixture of χ2H−1 and χ2H, where H is the number of moment conditions, as opposed to a χ2H−1. With more than two assets, this distribution lies between the χ2Hp and χ2H (p denotes the number of parameters). These results show that ignoring the lack of first‐order identification of the moment condition model leads to oversized tests with a possibly increasing overrejection rate with the number of assets. A Monte Carlo study illustrates these findings.  相似文献

The objective of the present study was to integrate the relative risk from mercury exposure to stream biota, groundwater, and humans in the Río Artiguas (Sucio) river basin, Nicaragua, where local gold mining occurs. A hazard quotient was used as a common exchange rate in probabilistic estimations of exposure and effects by means of Monte Carlo simulations. The endpoint for stream organisms was the lethal no‐observed‐effect concentration (NOECs), for groundwater the WHO guideline and the inhibitory Hg concentrations in bacteria (IC), and for humans the tolerable daily intake (TDI) and the benchmark dose level with an uncertainty factor of 10 (BMDLs0.1). Macroinvertebrates and fish in the contaminated river are faced with a higher risk to suffer from exposure to Hg than humans eating contaminated fish and bacteria living in the groundwater. The river sediment is the most hazardous source for the macroinvertebrates, and macroinvertebrates make up the highest risk for fish. The distribution of body concentrations of Hg in fish in the mining areas of the basin may exceed the distribution of endpoint values with close to 100% probability. Similarly, the Hg concentration in cord blood of humans feeding on fish from the river was predicted to exceed the BMDLs0.1 with about 10% probability. Most of the risk to the groundwater quality is confined to the vicinity of the gold refining plants and along the river, with a probability of about 20% to exceed the guideline value.  相似文献

One‐third of the annual cases of listeriosis in the United States occur during pregnancy and can lead to miscarriage or stillbirth, premature delivery, or infection of the newborn. Previous risk assessments completed by the Food and Drug Administration/the Food Safety Inspection Service of the U.S. Department of Agriculture/the Centers for Disease Control and Prevention (FDA/USDA/CDC)( 1 ) and Food and Agricultural Organization/the World Health Organization (FAO/WHO)( 2 ) were based on dose‐response data from mice. Recent animal studies using nonhuman primates( 3 , 4 ) and guinea pigs( 5 ) have both estimated LD50s of approximately 107 Listeria monocytogenes colony forming units (cfu). The FAO/WHO( 2 ) estimated a human LD50 of 1.9 × 106 cfu based on data from a pregnant woman consuming contaminated soft cheese. We reevaluated risk based on dose‐response curves from pregnant rhesus monkeys and guinea pigs. Using standard risk assessment methodology including hazard identification, exposure assessment, hazard characterization, and risk characterization, risk was calculated based on the new dose‐response information. To compare models, we looked at mortality rate per serving at predicted doses ranging from 10?4 to 1012 L. monocytogenes cfu. Based on a serving of 106 L. monocytogenes cfu, the primate model predicts a death rate of 5.9 × 10?1 compared to the FDA/USDA/CDC (fig. IV‐12)( 1 ) predicted rate of 1.3 × 10?7. Based on the guinea pig and primate models, the mortality rate calculated by the FDA/USDA/CDC( 1 ) is underestimated for this susceptible population.  相似文献

Protection motivation theory states individuals conduct threat and coping appraisals when deciding how to respond to perceived risks. However, that model does not adequately explain today's risk culture, where engaging in recommended behaviors may create a separate set of real or perceived secondary risks. We argue for and then demonstrate the need for a new model accounting for a secondary threat appraisal, which we call secondary risk theory. In an online experiment, 1,246 participants indicated their intention to take a vaccine after reading about the likelihood and severity of side effects. We manipulated likelihood and severity in a 2 × 2 between‐subjects design and examined how well secondary risk theory predicts vaccination intention compared to protection motivation theory. Protection motivation theory performed better when the likelihood and severity of side effects were both low (R2 = 0.30) versus high (R2 = 0.15). In contrast, secondary risk theory performed similarly when the likelihood and severity of side effects were both low (R2 = 0.42) or high (R2 = 0.45). But the latter figure is a large improvement over protection motivation theory, suggesting the usefulness of secondary risk theory when individuals perceive a high secondary threat.  相似文献

Through the use of case‐control analyses and quantitative microbial risk assessment (QMRA), relative risks of transmission of cryptosporidiosis have been evaluated (recreational water exposure vs. drinking water consumption) for a Canadian community with higher than national rates of cryptosporidiosis. A QMRA was developed to assess the risk of Cryptosporidium infection through the consumption of municipally treated drinking water. Simulations were based on site‐specific surface water contamination levels and drinking water treatment log10 reduction capacity for Cryptosporidium. Results suggested that the risk of Cryptosporidium infection via drinking water in the study community, assuming routine operation of the water treatment plant, was negligible (6 infections per 1013 persons per day—5th percentile: 2 infections per 1015 persons per day; 95th percentile: 3 infections per 1012 persons per day). The risk is essentially nonexistent during optimized, routine treatment operations. The study community achieves between 7 and 9 log10Cryptosporidium oocyst reduction through routine water treatment processes. Although these results do not preclude the need for constant vigilance by both water treatment and public health professionals in this community, they suggest that the cause of higher rates of cryptosporidiosis are more likely due to recreational water contact, or perhaps direct animal contact. QMRA can be successfully applied at the community level to identify data gaps, rank relative public health risks, and forecast future risk scenarios. It is most useful when performed in a collaborative way with local stakeholders, from beginning to end of the risk analysis paradigm.  相似文献

Mycobacterium avium subspecies paratuberculosis (MAP) causes chronic inflammation of the intestines in humans, ruminants, and other species. It is the causative agent of Johne's disease in cattle, and has been implicated as the causative agent of Crohn's disease in humans. To date, no quantitative microbial risk assessment (QMRA) for MAP utilizing a dose‐response function exists. The objective of this study is to develop a nested dose‐response model for infection from oral exposure to MAP utilizing data from the peer‐reviewed literature. Four studies amenable to dose‐response modeling were identified in the literature search and optimized to the one‐parameter exponential or two‐parameter beta‐Poisson dose‐response models. A nesting analysis was performed on all permutations of the candidate data sets to determine the acceptability of pooling data sets across host species. Three of four data sets exhibited goodness of fit to at least one model. All three data sets exhibited good fit to the beta‐Poisson model, and one data set exhibited goodness of fit, and best fit, to the exponential model. Two data sets were successfully nested using the beta‐Poisson model with parameters α = 0.0978 and N50 = 2.70 × 102 CFU. These data sets were derived from sheep and red deer host species, indicating successful interspecies nesting, and demonstrate the highly infective nature of MAP. The nested dose‐response model described should be used for future QMRA research regarding oral exposure to MAP.  相似文献

Previous experimental research provides evidence that a familiar risk comparison within a risk ladder is understood by low‐ and high‐numerate individuals. It especially helps low numerates to better evaluate risk. In the present study, an eye tracker was used to capture individuals’ visual attention to a familiar risk comparison, such as the risk associated with smoking. Two parameters of information processing—efficiency and level—were derived from visual attention. A random sample of participants from the general population (N= 68) interpreted a given risk level with the help of the risk ladder. Numeracy was negatively correlated with overall visual attention on the risk ladder (rs=?0.28, p= 0.01), indicating that the lower the numeracy, the more the time spent looking at the whole risk ladder. Numeracy was positively correlated with the efficiency of processing relevant frequency (rs= 0.34, p < 0.001) and relevant textual information (rs= 0.34, p < 0.001), but not with the efficiency of processing relevant comparative information and numerical information. There was a significant negative correlation between numeracy and the level of processing of relevant comparative risk information (rs=?0.21, p < 0.01), indicating that low numerates processed the comparative risk information more deeply than the high numerates. There was no correlation between numeracy and perceived risk. These results add to previous experimental research, indicating that the smoking risk comparison was crucial for low numerates to evaluate and understand risk. Furthermore, the eye‐tracker method is promising for studying information processing and improving risk communication formats.  相似文献

Many health‐related decisions require choosing between two options, each with risks and benefits. When presented with such tradeoffs, people often make choices that fail to align with scientific evidence or with their own values. This study tested whether risk communication and values clarification methods could help parents and guardians make evidence‐based, values‐congruent decisions about children's influenza vaccinations. In 2013–2014 we conducted an online 2×2 factorial experiment in which a diverse sample of U.S. parents and guardians (n = 407) were randomly assigned to view either standard information about influenza vaccines or risk communication using absolute and incremental risk formats. Participants were then either presented or not presented with an interactive values clarification interface with constrained sliders and dynamic visual feedback. Participants randomized to the risk communication condition combined with the values clarification interface were more likely to indicate intentions to vaccinate (β = 2.10, t(399) = 2.63, p < 0.01). The effect was particularly notable among participants who had previously demonstrated less interest in having their children vaccinated against influenza (β = –2.14, t(399) = –2.06, p < 0.05). When assessing vaccination status reported by participants who agreed to participate in a follow‐up study six months later (n = 116), vaccination intentions significantly predicted vaccination status (OR = 1.66, 95%CI (1.13, 2.44), p < 0.05) and rates of informed choice (OR = 1.51, 95%CI (1.07, 2.13), p < 0.012), although there were no direct effects of experimental factors on vaccination rates. Qualitative analysis suggested that logistical barriers impeded immunization rates. Risk communication and values clarification methods may contribute to increased vaccination intentions, which may, in turn, predict vaccination status if logistical barriers are also addressed.  相似文献

We develop and apply a judgment‐based approach to selecting robust alternatives, which are defined here as reasonably likely to achieve objectives, over a range of uncertainties. The intent is to develop an approach that is more practical in terms of data and analysis requirements than current approaches, informed by the literature and experience with probability elicitation and judgmental forecasting. The context involves decisions about managing forest lands that have been severely affected by mountain pine beetles in British Columbia, a pest infestation that is climate‐exacerbated. A forest management decision was developed as the basis for the context, objectives, and alternatives for land management actions, to frame and condition the judgments. A wide range of climate forecasts, taken to represent the 10–90% levels on cumulative distributions for future climate, were developed to condition judgments. An elicitation instrument was developed, tested, and revised to serve as the basis for eliciting probabilistic three‐point distributions regarding the performance of selected alternatives, over a set of relevant objectives, in the short and long term. The elicitations were conducted in a workshop comprising 14 regional forest management specialists. We employed the concept of stochastic dominance to help identify robust alternatives. We used extensive sensitivity analysis to explore the patterns in the judgments, and also considered the preferred alternatives for each individual expert. The results show that two alternatives that are more flexible than the current policies are judged more likely to perform better than the current alternatives on average in terms of stochastic dominance. The results suggest judgmental approaches to robust decision making deserve greater attention and testing.  相似文献

The aim of this study was to develop a reliable and valid measure of hurricane risk perception. The utility of such a measure lies in the need to understand how people make decisions when facing an evacuation order. This study included participants located within a 15‐mile buffer of the Gulf and southeast Atlantic U.S. coasts. The study was executed as a three‐wave panel with mail surveys in 2010–2012 (T0 baseline N = 629, 56%; T1 retention N = 427, 75%; T2 retention N = 350, 89%). An inventory based on the psychometric model was developed to discriminate cognitive and affective perceptions of hurricane risk, and included open‐ended responses to solicit additional concepts in the T0 survey. Analysis of the T0 data modified the inventory and this revised item set was fielded at T1 and then replicated at T2. The resulting scales were assessed for validity against existing measures for perception of hurricane risk, dispositional optimism, and locus of control. A measure of evacuation expectation was also examined as a dependent variable, which was significantly predicted by the new measures. The resulting scale was found to be reliable, stable, and largely valid against the comparison measures. Despite limitations involving sample size, bias, and the strength of some reliabilities, it was concluded that the measure has potential to inform approaches to hurricane preparedness efforts and advance planning for evacuation messages, and that the measure has good promise to generalize to other contexts in natural hazards as well as other domains of risk.  相似文献

We developed a risk assessment of human salmonellosis associated with consumption of alfalfa sprouts in the United States to evaluate the public health impact of applying treatments to seeds (0–5‐log10 reduction in Salmonella ) and testing spent irrigation water (SIW) during production. The risk model considered variability and uncertainty in Salmonella contamination in seeds, Salmonella growth and spread during sprout production, sprout consumption, and Salmonella dose response. Based on an estimated prevalence of 2.35% for 6.8 kg seed batches and without interventions, the model predicted 76,600 (95% confidence interval (CI) 15,400 – 248,000) cases/year. Risk reduction (by 5 ‐ to 7‐fold) predicted from a 1‐log10 seed treatment alone was comparable to SIW testing alone, and each additional 1‐log10 seed treatment was predicted to provide a greater risk reduction than SIW testing. A 3‐log10 or a 5‐log10 seed treatment reduced the predicted cases/year to 139 (95% CI 33 – 448) or 1.4 (95% CI <1 – 4.5), respectively. Combined with SIW testing, a 3‐log10 or 5‐log10 seed treatment reduced the cases/year to 45 (95% CI 10–146) or <1 (95% CI <1 – 1.5), respectively. If the SIW coverage was less complete (i.e., less representative), a smaller risk reduction was predicted, e.g., a combined 3‐log10 seed treatment and SIW testing with 20% coverage resulted in an estimated 92 (95% CI 22 – 298) cases/year. Analysis of alternative scenarios using different assumptions for key model inputs showed that the predicted relative risk reductions are robust. This risk assessment provides a comprehensive approach for evaluating the public health impact of various interventions in a sprout production system.  相似文献

The three classic pillars of risk analysis are risk assessment (how big is the risk and how sure can we be?), risk management (what shall we do about it?), and risk communication (what shall we say about it, to whom, when, and how?). We propose two complements as important parts of these three bases: risk attribution (who or what addressable conditions actually caused an accident or loss?) and learning from experience about risk reduction (what works, and how well?). Failures in complex systems usually evoke blame, often with insufficient attention to root causes of failure, including some aspects of the situation, design decisions, or social norms and culture. Focusing on blame, however, can inhibit effective learning, instead eliciting excuses to deflect attention and perceived culpability. Productive understanding of what went wrong, and how to do better, thus requires moving past recrimination and excuses. This article identifies common blame‐shifting “lame excuses” for poor risk management. These generally contribute little to effective improvements and may leave real risks and preventable causes unaddressed. We propose principles from risk and decision sciences and organizational design to improve results. These start with organizational leadership. More specifically, they include: deliberate testing and learning—especially from near‐misses and accident precursors; careful causal analysis of accidents; risk quantification; candid expression of uncertainties about costs and benefits of risk‐reduction options; optimization of tradeoffs between gathering additional information and immediate action; promotion of safety culture; and mindful allocation of people, responsibilities, and resources to reduce risks. We propose that these principles provide sound foundations for improving successful risk management.  相似文献
